摘要
目的:探讨老年住院患者骨代谢情况及影响骨代谢指标的相关因素。方法:回顾性分析2016年3月-2019年3月山东中医药大学附属医院老年医学科300例骨代谢异常患者的血清甲状旁腺激素(Parathyroid Hormone,PTH)、骨钙素(Bone Gla Protein,BGP)、β-胶原特殊序列(β-collagen Special Sequence,β-CTx)、25-羟基维生素D[25-hydroxyitamin D,25(OH)D]、总Ⅰ型胶原氨基端延长肽(N-terminal Elongation Peptide of Total Type I Collagen,TPINP)检测结果。并分析性别、年龄和中医证型对血清骨代谢水平的影响。结果:男性血清BGP、β-CTx、TPINP水平明显低于女性,差异具有统计学意义(P<0.05)。血清PTH水平与患者年龄呈正相关(r=0.015,P<0.05),BGP、25(OH)D水平与患者年龄呈负相关,相关系数差异有统计学意义(分别为r=-0.153,r=-0.293,P<0.05)。患者中医证型分布以气虚血瘀证为主,气虚血瘀证与其他证型各检测值比较,均有显著差异(P<0.05)。结论:老年住院患者的血清骨代谢水平受性别、年龄和中医证型等多种因素的影响。
Objective:To investigate the bone metabolism of elderly inpatients and the related factors affecting bone metabolism indexes.Methods:The test results of PTH,BGP,β-CTx,25(OH)D,and TPINP of 300 elderly inpatients with abnormal bone metabolism from March 2016 to March 2019 were retrospectively analyzed.And the influences of gender,age and TCM syndrome type on serum bone metabolism level were analyzed.Results:The serum BGP,β-CTx and TPINP levels in male patients were significantly lower than those in female patients,and the difference was statistically significant(P<0.05).Serum PTH level was positively correlated with age(r=0.015,P<0.05),BGP and 25(OH)D levels were negatively correlated with age,and the correlation coefficients were statistically significant(r=-0.153,r=0.293,P<0.05).TCM syndrome types were mainly the Qi(气)deficiency and blood stasis syndrome,and there were significant differences among the Qi deficiency and blood stasis syndrome and other syndrome types in detection values(P<0.05).Conclusion:The level of serum bone metabolism in elderly inpatients is affected by gender,age,TCM syndrome type and so on.
