基于肠道菌群探析麻芍平喘汤对小儿支气管哮喘炎症因子及ACTH、CORT的影响

Effect of MaShao Pingchuan Decoction(麻芍平喘汤)on Inflammatory Factors,ACTH and CORT in Children with Bronchial Asthma Based on Intestinal Flora

目的基于肠道菌群探析麻芍平喘汤对小儿支气管哮喘患者炎症因子及促肾上腺皮质激素(ACTH)、皮质酮(CORT)的影响。方法纳入96例小儿支气管哮喘患者,随机分为两组,每组48例,对照组采用常规西医治疗,治疗组在对照组基础上加用麻芍平喘汤治疗,两组均治疗两周,观察两组患者治疗后临床总有效率,临床症状积分,炎症因子水平,ACTH水平、CORT水平及肠道菌群含量。结果治疗2周后,治疗组总有效率为91.66%(44/48),显著高于对照组77.08%(37/48),差异具有统计学意义(P<0.05);治疗组患儿哮鸣、胸闷、喘息、咳痰改善情况均优于对照组,差异具有统计学意义(P<0.05);两组患儿血清白介素(IL)-6、肿瘤坏死因子α(TNF-α)、ACTH、CORT水平均降低(P<0.05),IL-4水平升高(P<0.05),治疗组均优于对照组(P<0.05);两组双歧杆菌、乳酸杆菌含量较治疗前升高(P<0.05),大肠杆菌含量较治疗前降低(P<0.05),治疗组均优于对照组(P<0.05)。结论麻芍平喘汤治疗小儿支气管哮喘效果显著,可缓解患儿临床症状,改善患儿炎症因子水平,安全有效。

Objective To analyze the effects of Mashao Pingchuan Decoction(麻芍平喘汤)on inflammatory factors,ACTH and CORT in children with bronchial asthma based on the intestinal flora.Methods Ninety-six children with bronchial asthma were enrolled and randomly divided into two groups,each with 48 cases.The control group was treated with conventional Western medicine,and the treatment group was treated with Mashao Pingchuan Decoction on the basis of the control group.Both groups were treated for two weeks.The total clinical effective rate,clinical symptom score,the levels of inflammatory factors,adrenocorticotropic hormone(ACTH)and corticosterone(CORT)and the intestinal flora content after treatment in the two groups.Results After 2 weeks of treatment,the total effective rate of the treatment group was 91.66%(44/48),which was significantly higher than that of the control group 77.08%(37/48),and the difference was statistically significant(P<0.05).The improvement of wheezing,chest tightness,gasping and expectoration in the treatment group were all better than those of the control group,and the difference was statistically significant(P<0.05).The levels of interleukin-6(IL-6),tumor necrosis factor alpha(TNF-α),ACTH and CORT were decreased(P<0.05).The IL-4 levels were increased(P<0.05)and the IL-4 level in the treatment group was better than that of the control group(P<0.05).The contents of bifidobacteria and lactobacilli in the two groups were higher than those before treatment(P<0.05),and the E.coli content was lower than that before treatment(P<0.05).Those of the treatment group were better than those of the control group(P<0.05).Conclusion Mashao Pingchuan Decoction is effective in treating bronchial asthma in children.It can relieve the clinical symptoms and improve the levels of inflammatory factors in children.It is safe and effective.
