摘要
目的探讨不同频率重复经颅磁刺激对失语症图命名作用。方法选取2016年5月-2018年10月河北省人民医院收治的失语症患者60例,根据随机数表法分为低频率组和高频率组,各30例,低频率组给予低频重复经颅磁刺激治疗,刺激点为患者右侧半球Broca对应区,运动阈值的80%为刺激强度,频率为1Hz,高频率组给予高频重复经颅磁刺激治疗,刺激点为患者左侧半球Broca对应区,频率为5Hz,每周治疗5d,共治疗2周。观察两组临床疗效,并对两组治疗前后视图命名得分及反应时间进行比较。结果低频率组总有效率(96.67%)高于高频率组(73.33%),差异有统计学意义(P<0.05)。治疗前两组视图命名得分和视图命名反应时间差异无统计学意义(P>0.05);治疗后两组视图命名得分均升高,且低频率组得分升高程度较高频率组明显,两组视图命名反应时间均减少,且低频率组反应时间多于高视频组,差异有统计学意义(P<0.05)。结论低频率和高频率重复经颅磁刺激对失语症均有一定治疗效果,低频率重复经颅磁刺激临床疗效较好,视图命名得分较高,但高频率重复经颅磁刺激视图命名反应时间较少,值得临床进一步研究。
Objective To investigate the effect of repetitive transcranial magnetic stimulation with different frequencies on visual naming in aphasia patients.Methods 60 patients with aphasia admitted to Hebei General Hospital from May 2016 to October 2018 were selected.They were divided into low frequency group and high frequency group according to the random number table method,with 30 cases in each group.The low frequency group was treated with low frequency repetitive transcranial magnetic stimulation at a frequency of 1 Hz,the stimulation point was the Broca corresponding area in the right hemisphere of the patient,the 80%of the exercise threshold was the intensity of stimulation.The high frequency group was given high frequency repetitive transcranial magnetic stimulation at a frequency of 5 Hz,the stimulation point was the Broca corresponding area in the left hemisphere of the patient,5 days per week for a total of 2 weeks.The clinical efficacy of the two groups was observed,and the visual naming scores and reaction time of the two groups were compared before and after treatment.Results The total efficiency of the low frequency group(96.67%)was higher than 73.33%of the high frequency group,with statistically significant difference(P<0.05).Before treatment,there was no significant difference in visual naming scores and reaction time between the two groups(P>0.05).After treatment,the score of visual naming in both groups increased,and the scores of low frequency group were higher than those of high frequency group.The reaction time of visual naming in both groups decreased,and the response time in low frequency group was more than that in high frequency group,with statistically significant difference(P<0.05).Conclusion Both low frequency and high frequency repetitive transcranial magnetic stimulation have certain therapeutic effects on aphasia.Low frequency repetitive transcranial magnetic stimulation has a better clinical effect and higher scores of visual naming,but high frequency repetitive transcranial magnetic stimulation has less response time for visual naming,which is worthy of further clinical research.
