不同加压弹力袜对预防老年脑卒中患者下肢深静脉血栓形成的效果分析

Effect Analysis of Different Compression Elastic Socks on Preventing Deep Venous Thrombosis of Lower Limbs of Elderly Stroke Patients

目的探讨不同加压弹力袜对预防老年脑卒中患者下肢深静脉血栓(DVT)形成的效果。方法本次试验选取广州市第一人民医院2018年6月-2020年8月收治的78例老年脑卒中患者进行研究,按照随机抽样法分为各26例的三组,在常规护理的基础上分别给予15mmHg~20mmHg过膝低压弹力袜,20mmHg~30mmHg膝下中压弹力袜以及15mmHg~20mmHg膝下低压弹力袜,经过一段时间的护理之后分析使用不同弹力袜的差异。结果三组患者在DVT的发生概率当中不具备统计学含义(P>0.05);膝下低压组比过膝低压组、膝下中压组双下肢皮肤温度更低,股浅静脉血流速度及腘窝静脉血流速度更慢,差异具有统计学意义(P<0.05);膝下低压组比过膝低压组、膝下中压组PT、APTT更长,差异具有统计学意义(P<0.05);三组使用弹力袜不良情况具有差异(P<0.05);膝下低压组比过膝低压组、膝下中压组临床依从性更高,差异具有统计学意义(P<0.05)。结论膝下低压弹力袜效果更好,在结合患者病情的前提下推荐选用,可降低老年脑卒中患者下肢深静脉血栓形成率,减缓静脉血流速度,提升临床依从性,延长凝血酶原时间,实践价值较高。

Objective To explore effect of different compression elastic socks on prevention of deep venous thrombosis(DVT)of lower limbs of elderly stroke patients.Methods The paper chose 78 elderly stroke patients admitted to Guangzhou City the First People’s Hospital from June 2018 to August 2020,and divided them into three groups randomly,with 26 cases in each group.on the basis of routine nursing,they was treated with 15mmhg-20mmhg knee low-pressure elastic socks,20mmhg-30mmhg knee low-pressure elastic socks and 15mmhg-20mmhg knee low-pressure elastic socks respectively.After a period of nursing,difference of different elastic socks was analyzed.Results Three groups of patients did not showed statistical significance in incidence of DVT(P>0.05).Skin temperature of both lower limbs was lower,and superficial femoral vein blood flow velocity and popliteal vein blood flow velocity were slower in subknee hypotension group than over knee hypotension group and subknee hypotension group,(P<0.05).PT and APTT were longer in subgenicular hypotension group than over knee hypotension group and subgenicular middle pressure group,difference was statistically significant(P<0.05),Bad situation of elastic socks among three groups was different,(P<0.05).Clinical compliance of subknee depression group was higher than over knee depression group and subknee middle pressure group,difference was statistically significant(P<0.05).Conclusion Low-pressure elastic socks under knees can achieve better effect,which can be applied under combining condition of patients.It can reduce rate of deep venous thrombosis of lower limbs of elderly stroke patients,slow down velocity of venous blood flow,improve clinical compliance,and prolong prothrombin time,which has high practical value.
