小青龙汤加减在慢性阻塞性肺疾病急性加重期痰浊阻肺证治疗中的临床应用

Clinical Application of Xiaoqinglong Decoction in the Treatment of Chronic Obstructive Pulmonary Disease Acute Exacerbation of Phlegm Turbidity Obstructing the Lung Syndrome

目的观察小青龙汤加减在慢性阻塞性肺疾病(COPD)急性加重期痰浊阻肺证治疗中的应用。方法本院收治的COPD急性加重期痰浊阻肺证患者82例为研究对象,随机分为治疗组和对照组,每组各41例。对照组给予COPD急性加重期常规治疗,治疗组在对照组基础上给予小青龙汤加减治疗。比较两组中医证候积分、中医证候疗效、血气指标和不良反应情况。结果治疗前,两组中医证候积分和血气指标无显著差别(P>0.05)。治疗后,治疗组咳嗽、咳痰、喘息、胸闷等单项积分及总分均明显低于对照组(P<0.05),治疗组PaO_(2)显著高于对照组(P<0.05),PaCO_(2)显著低于对照组(P<0.05)。治疗期间两组均未发现明显不良反应。结论小青龙汤加减治疗COPD急性加重期痰浊阻肺证疗效显著,可改善肺功能和临床症状体征,并且用药安全。

Objective Observe the application of Xiaoqinglong Decoction in the treatment of chronic obstructive pulmonary disease(COPD)acute exacerbation of phlegm turbidity obstructing the lung syndrome.Methods Eighty-two patients with phlegm turbidity obstructing lung syndrome in the acute exacerbation of COPD in our hospital were the research objects,and they were randomly divided into treatment group and control group,with 41 cases in each group.The control group was given conventional treatment for the acute exacerbation of COPD,and the treatment group was given Xiaoqinglong Decoction on the basis of the control group.The scores of TCM syndromes,the efficacy of TCM syndromes,blood gas indexes and adverse reactions were compared between the two groups.Results Before treatment,there was no significant difference in TCM syndrome scores and blood gas indexes between the two groups(P>0.05).After treatment,the individual items and total scores of cough,sputum,wheezing,chest tightness in the treatment group were significantly lower than those in the control group(P<0.05),and the PaO_(2)of the treatment group was significantly higher than that of the control group(P<0.05),PaCO_(2)was significantly lower than the control group(P<0.05).During the treatment period,no obvious adverse reactions were found in both groups.Conclusion Xiaoqinglong Decoction has a significant effect on the treatment of phlegm turbidity obstructing the lung syndrome in the acute exacerbation of COPD,which can improve lung function and clinical symptoms and signs,and it is safe to use.
