摘要
目的分析重症脑出血患者ICU院内肺部感染的原因,评价其治疗效果。方法择取昆明市第一人民医院2017年5月-2020年6月接诊的76例重症脑出血患者予以研究,将未出现肺部感染患者作为对照组(n=38),将出现院内肺部感染患者作为观察组(n=38);对比分析两组患者年龄、吸烟史、气管插管等情况,对两组患者选择相同治疗方式,比较治疗疗效。结果比较两组临床各指标:与对照组比较,观察组平均年龄高,进入ICU时间、呼吸机使用时间长,气管切管率、吸烟率高,差异具有统计学意义(P<0.05)。比较两组治疗疗效:与对照组比较,观察组治疗疗效显著偏高,差异具有统计学意义(P<0.05)。比较两组并发症发生率:与对照组比较,观察组并发症发生率低,差异具有统计学意义(P<0.05)。结论重症脑出血患者出现感染的原因包括进入ICU时间、呼吸机使用时间、是否吸烟等,因此临床医护人员需要对肺部感染原因进行分析,采取针对性措施进行干预,从而降低肺部感染发生率。
Objective To analyze the causes of nosocomial pulmonary infection in ICU patients with severe cerebral hemorrhage and evaluate its therapeutic effect.Methods Seventy-six patients with severe intracerebral hemorrhage in our hospital from May 2017 to June 2020 were selected for the study.The patients without pulmonary infection were selected as the control group(n=38),and the patients with nosocomial pulmonary infection were selected as the observation group(n=38).The age,smoking history and endotracheal intubation of the two groups were compared and analyzed.The same treatment method was selected for the two groups,and the therapeutic effect was compared.Results The clinical indexes of the two groups were compared:Compared with the control group,the average age of the observation group was higher,the ICU time,the ventilator use time was longer,the tracheotomy rate and smoking rate were higher,the difference was statistically significant(P<0.05).The therapeutic effect of the two groups was compared:Compared with the control group,the therapeutic effect of the observation group was significantly higher,the difference was statistically significant(P<0.05).Compared the incidence of complications between the two groups:Compared with the control group,the incidence of complications in the observation group was lower,the difference was statistically significant(P<0.05).Conclusion The causes of infection in patients with severe cerebral hemorrhage include the time of entering ICU,the time of using ventilator,whether smoking,etc.,so the clinical medical staff need to analyze the causes of pulmonary infection and take targeted measures for intervention,so as to reduce the incidence of pulmonary infection.
