共同照护模式对2型糖尿病患者自我管理的效果分析

Analysis of the Effect of Shared Care Model on Self-management of Patients with Type 2 Diabetes Mellitus

目的探讨以医师-营养师-糖尿病专科护士为基本管理团队的糖尿病共同照护模式参与2型糖尿病管理的效果。方法于2019年6月至2020年9月收治120例2型糖尿病患者为研究对象,随机分组,各60例,对照组采用常规治疗+健康教育+护理,观察组采用糖尿病共同照护模式。两组干预前后临床指标(体质指数[BMI]、糖化血红蛋白、低密度脂蛋白胆固醇[LDL-C]、尿微量白蛋白)、糖尿病知识评分、自我效能、自我管理能力及生活质量比较。结果两组干预前BMI、HbAIc、LDL-C、ALB、SDSCA-6、C-DMSES、DKT、DSQL差异无统计学意义(P>0.05),经不同干预方案,观察组BMI、HbAIc、LDL-C、ALB、DSQL低于对照组,观察组SDSCA、C-DMSES、DKT高于对照组,差异有统计学意义(P<0.05)。结论针对2型糖尿病患者采用共同照护模式可有效控制其BMI、HbAIc、LDL-C、ALB,有利于提高患者的自我管理及效能水平,提升患者对疾病的认知,提高患者的生活质量,值得临床推广。

Objective To explore the effect of diabetes shared care model with physician dietitian diabetes specialist nurse as the basic management team to participate in the management of type 2 diabetes mellitus.Methods From June 2019 to September 2020,120 patients with type 2 diabetes were randomly divided into two groups,60 cases in each group.The clinical indexes(body mass index[BMI],glycosylated hemoglobin,low density lipoprotein cholesterol[LDL-C],urinary microalbumin),diabetes knowledge score,self-efficacy,self-management ability and quality of life were compared between the two groups before and after intervention.Results There was no statistical difference in BMI,HbAIc,LDL-C,ALB,SDSCA-6,C-DMSES,DKT,DSQL between the two groups before intervention(P>0.05).After different intervention programs,BMI,HbAIc,LDL-C,ALB and DSQL of the observation group were lower than those in the control group,and the observation group’s SDSCA,C-DMSES and DKT were higher than those in the control group(P<0.05).Conclusion For patients with type 2 diabetes mellitus,the use of shared care mode can effectively control their BMI,HBAIC,LDL-C,ALB,which is conducive to the improvement of the level of self-management and efficacy of patients,enhancing patients’awareness of the disease,and improving the quality of life of patients,and it is worthy of clinical promotion.
