术前咀嚼口香糖对妇科腹腔镜手术患者术后疼痛、胃肠道功能和并发症的影响

Effects of Preoperative Chewing Gum on Postoperative Pain,Gastrointestinal Function and Complications in Patients Undergoing Gynecological Laparoscopic Surgery

目的探究术前咀嚼口香糖对妇科腹腔镜手术患者术后疼痛、胃肠道功能和并发症的影响。方法选择2019年6月至2020年6月于罗定市人民医院妇科住院行腹腔镜手术患者56例作为研究对象,利用随机数字表法将其分为对照组(28例)和观察组(28例)。对照组采用常规围手术期管理模式,观察组在对照组基础上联合术前咀嚼口香糖,比较两组疼痛评分[术后2h、4h、8h、12h、24h疼痛视觉模拟评分(VAS评分)]、术后胃肠道功能(首次排气、排便时间)和并发症。结果两组麻醉时间、手术时间和镇痛药物用量差异无统计学意义(P>0.05);观察组术后2h和4h VAS评分低于对照组(P<0.05),两组术后8h,12h,24hVAS评分差异无统计学意义(P>0.05);观察组首次排气时间和首次排便时间均早于对照组(P<0.05);观察组恶心、腹胀发生率低于对照组(P<0.05),两组呕吐和咽喉痛差异无统计学意义(P>0.05)。结论术前咀嚼口香糖通过兴奋副交感神经降低妇科腹腔镜手术患者术后疼痛,有助于促进胃肠道功能恢复,降低并发症。

Objective To explore the effect of chewing gum before operation on gastrointestinal function and complications of gynecological laparoscopic surgery.Methods From June 2019 to June 2020,56 patients with laparoscopic surgery in our hospital were selected and randomly divided into control group(28 cases)and observation group(28 cases).The control group received routine perioperative management mode,while the observation group chewed gum before operation on the basis of the control group.The VAS score at 2h,4h,8h,12h,24h after operation,gastrointestinal function(first exhaust,defecation time)and complications were compared between the two groups.Results There was no significant difference in anesthesia time,operation time and analgesic dosage between the two groups(P>0.05);2h and 4h VAS score after operation in the observation group were lower than that of the control group(P<0.05),and there was no significant difference in the VAS score between the two groups 8h,12h,24h after operation(P>0.05);the time of first exhaust and the first defecation in the observation group were earlier than those in the control group(P<0.05);the incidence of nausea and abdominal distension in the observation group was lower than that in the control group(P<0.05),and there was no significant difference in vomiting and sore throat between the two groups(P>0.05).Conclusion Chewing gum before operation can reduce the pain of after gynecological laparoscopic surgery by stimulating parasympathetic nerve,which is helpful to promote the recovery of gastrointestinal function and reduce complications.
