摘要
目的分析不同呼气末二氧化碳对小儿先心病患者围术期脑氧饱和度的影响。方法将我院2018年1月至2020年1月收治的45例先天性心脏病患儿纳入至本次研究,根据呼气末二氧化碳参数随机分为甲组(呼气末二氧化碳值为25~35 mmHg)、乙组(呼气末二氧化碳值为35~45 mmHg)、丙组(呼气末二氧化碳值为45~50 mmHg),每组15例。观察并比较三组临床效果。结果T0~1时,三组脑氧饱和度无显著差异(P>0.05);T2~3时,丙组、乙组脑氧饱和度明显高于甲组,组间对比差异有统计学意义(P<0.05);T4~5时,丙组脑氧饱和度高于乙组(P<0.05)。结论呼气末二氧化碳与脑氧饱和度呈正相关关系,在先天性心脏病患儿行手术治疗时,适当增加呼气末二氧化碳可有效提高患儿的脑氧饱和度,促使脑氧供需平衡。
Objective To analyze the effect of different end expiratory carbon dioxide on perioperative cerebral oxygen saturation in children with congenital heart disease.Methods Forty-five children with congenital heart disease admitted to our hospital from January 2018 to January 2020 were included in this study.According to the parameters of end expiratory carbon dioxide,they were randomly divided into group A(end expiratory carbon dioxide value 25~35 mmHg),group B(end expiratory carbon dioxide value 35~45 mmHg)and group C(end expiratory carbon dioxide value 45~50 mmHg),15 cases in each group.Observe and compare the clinical effect of three groups.Results At T0~1,there was no significant difference between the three groups(P>0.05).At T2~3,the brain oxygen saturation of group C and group B was significantly higher than that of group A,with statistical significance(P<0.05).At T4~5,the brain oxygen saturation of group C was higher than that of group B(P<0.05).Conclusion There is a positive correlation between end expiratory carbon dioxide and cerebral oxygen saturation.In the operation of children with congenital heart disease,increasing the end expiratory carbon dioxide properly can effectively improve the cerebral oxygen saturation and promote the balance of cerebral oxygen supply and demand.
