比较经皮椎间孔镜TESSYS技术与传统后路小切口椎板开窗髓核摘除术治疗腰椎间盘突出症的临床疗效

To Compare the Clinical Effect of Percutaneous Transforaminal Microsurgery(tessys) and Traditional Small Incision Laminectomy in the Treatment of Lumbar Disc Herniation

目的分析对比经皮椎间孔镜TESSYS技术与传统后路小切口椎板开窗髓核摘除术治疗腰椎间盘突出症的临床疗效。方法选取本院接收的腰椎间盘突出症患者50例,经住院单双病号分组,各25例,常规组行传统后路小切口椎板开窗髓核摘除术治疗,实验组行经皮椎间孔镜TESSYS技术治疗,对比两组临床疗效。结果实验组患者术中出血量明显少于常规组,手术切口、手术时间、术后卧床时间、住院时间明显短于常规组,随访1年手术效果优良率96.00%明显高于常规组76.00%(P<0.05),两组患者术后24h、术后1个月、术后3个月的VAS评分对比差异无统计学意义(P>0.05)。结论经皮椎间孔镜TESSYS技术治疗腰椎间盘突出症的临床疗效比传统后路小切口椎板开窗髓核摘除术更加显著,术后疼痛感更轻,腰椎功能恢复更快,手术安全性更高。

Objective To analyze and compare the clinical effect of percutaneous transforaminal microsurgery(tessys)and traditional posterior small incision laminectomy in the treatment of lumbar disc herniation.Methods 50 patients with lumbar disc herniation were divided into two groups,25 in each group.The conventional group was treated by posterior small incision laminectomy,and the experimental group was treated by transdermal endoscopic tessys.Results The bleeding volume in the experimental group was significantly less than that in the conventional group,and the incision,operation time,bed rest time and hospitalization time were significantly shorter than those in the conventional group.The excellent and good rate of operation effect in one year follow-up was 96.00%,which was significantly higher than 76.00%in the conventional group(P<0.05).There was no significant difference in VAS scores between the two groups at 24 hours,1 month and 3 months after operation(P>0.05)Conclusion The clinical effect of the treatment of lumbar disc herniation with tessys is more significant than that of the traditional posterior small incision laminectomy,with less pain,faster recovery of lumbar function and higher safety.
