负压封闭引流术联合外用生肌膏序贯应用治疗足部外伤性创面临床护理学研究

Nursing research on the sequential application of vacuum sealing and drainage combined with external Shengji ointment in the healing of traumatic foot wounds

目的探讨创伤性足部软组织缺损采用负压封闭引流技术(VSD)和生肌膏联合治疗的临床效果。方法60例足部软组织严重损伤患者随机分为观察组和对照组,每组30例。对照组患者单纯给予负压封闭引流术治疗,观察组患者给予负压封闭引流术和生肌膏联合治疗,对比两组患者治疗后第3天、7天、14天、21天、28天创面面积缩小率,对比两组患者VSD装置更换次数、创面愈合时间。结果观察组治疗后第14天(60.36±5.24)%、第21天(80.15±7.26)%、第28天(91.75±8.24)%创面面积缩小率与对照组治疗后第14天(45.28±3.18)%、第21天(62.57±5.19)%、第28天(70.17±6.57)%创面面积缩小率比较明显提高;观察组VSD装置更换次数(1.42±0.12)次与对照组(3.48±0.28)次比较明显减少;创面愈合时间(20.59±1.69)d与对照组(31.37±2.61)d比较明显缩短,观察组患者治疗3周后,肉芽组织评分(1.67±0.12)分、分泌物评分(1.54±0.15)分与对照组肉芽组织评分(2.22±0.17)分、分泌物评分(2.10±0.16)分比较明显降低,差异有统计学意义(P<0.05)。结论足部外伤性创面采用负压封闭引流术和外用生肌膏联合治疗,可以加快创面修复速度,缩短创面愈合时间,临床效果良好。

Objective To explore the clinical effect of vacuum sealing drainage(VSD)combined with Shengji ointment in the treatment of traumatic foot soft tissue defect.Methods 60 patients with severe foot soft tissue injury were randomly divided into the observation group and the control group,30 cases in each group.Patients in the control group were treated with VSD alone,while those in the observation group were treated with both VSD and Shengji cream.The wound surface reduction rate on the 3 rd,7 th,14 th,21 th and 28 th day after treatment,the replacement times of VSD device and the healing time of wound were compared between the two groups.Results The wound surface reduction rates in the observation group on the 14 th day,the 21 th day and the 28 th day were significantly higher than that in the control group(60.36±5.24)%vs(45.28±3.18)%;(80.15±7.26)%vs(62.57±5.19)%;(91.75±8.24)%vs(70.17±6.57)%;(P<0.05).The number of VSD device replacement in the observation group was significantly lower than that in the control group(1.42±0.12 vs 3.48±0.28),and the wound healing time was significantly shorter(20.59±1.69 d vs 31.37±2.61 d).After 3 weeks of treatment,the granulation tissue score,and secretion score in the observation group were significantly lower than those in the con trol group(1.67±0.12 vs 2.22±0.17;1.54±0.15 vs 2.10±0.16;P<0.05).Conclusions The combined use of VSD with external Shengji plaster can accelerate wound healing speed,shorten wound healing time.
