摘要
目的了解矮小症患儿维生素D水平及影响因素,为开展矮小症患儿维生素D缺乏的防治工作提供科学依据。方法采用方便抽样法,选取2022年四川省乐山市人民医院儿科628例矮小症患儿,收集患儿一般情况、体格及实验室检查相关资料,采用多因素Logistic逐步回归分析其维生素D水平的影响因素。结果矮小症患儿血清25-羟基维生素D水平M(P_(25),P_(75))为[27.86(22.31,35.88)ng/mL]。矮小症患儿中维生素D不足率为12.6%,缺乏率为0.9%,不足和缺乏率为13.5%。多因素Logistic逐步回归分析结果显示:以男性为参照,女性维生素D不足和缺乏率较高,OR值为1.730(95%CI:1.027~2.916);以年龄3~5岁的患儿为参照,9~11岁和12岁及以上的患儿维生素D不足和缺乏率较高,OR值分别为4.366(95%CI:2.152~8.859)和8.265(95%CI:3.629~18.824);以头胎患儿为参照,非头胎患儿的维生素D不足和缺乏率较高,OR值为2.663(95%CI:1.607~4.412)。结论矮小症患儿维生素D不足和缺乏情况较为严重,维生素D缺乏的防治工作需重点关注非头胎儿童、女性、农村、9岁及以上的儿童。
Objective To investigate the vitamin D levels and influencing factors in children with short stature,and to provide scientific basis for the prevention and treatment of vitamin D deficiency in children with short stature.Methods By convenient sampling method,628 children with short stature were selected from the pediatrics department of Leshan People's Hospital of Sichuan Province in 2022.The general condition,physical condition and laboratory examination of these children were collected.Multifactor Logistic stepwise regression was used to analyze their vitamin D levels and influencing factors.Results The serum 25(OH)D content M(P_(25),P_(75))in children with short stature was[27.86(22.31,35.88)]ng/mL.The rate of vitamin D insufficiency was 12.6%,the rate of deficiency was 0.9%,and the rate of insufficiency and deficiency was 13.5%.The results of multifactor Logistic stepwise regression analysis were as follows.Compared with male children with short stature,girls had a higher rate of vitamin D insufficiency and deficiency,with an OR value of 1.730(95%CI:1.027-2.916).Compared with children aged 3-5 years,children aged 9-11 years and 12 years and older had a higher rate of vitamin D insufficiency and deficiency,with OR values of 4.366(95%CI:2.152-8.859)and 8.265(95%CI:3.629-18.824),respectively.Compared with the first-born children,the non-first-born children had a higher prevalence of vitamin D insufficiency and deficiency,with an OR value of 2.663(95%CI:1.607-4.412).Conclusion Vitamin D insufficiency and deficiency is more severe in children with short stature.Prevention and treatment of vitamin D deficiency should tocuson non-first-born children,girls,rural children,and children aged 9 years of age and older.
