健康生活方式对代谢综合征发病影响的队列研究 被引量:3

Impact of healthy lifestyle on metabolic syndrome: a cohort study

摘要 目的探讨健康生活方式对代谢综合征(MS)发病风险的影响,为MS的生活方式干预提供参考依据。方法数据来源于贵州省自然人群队列研究,在基线调查的9280人中,排除不符合纳入标准、失访及死亡人数,最终将4663人纳入分析。对8种健康生活方式:从不吸烟、从不饮酒、蔬菜水果摄入≥400 g/d、烹调油摄入≤30 g/d、食盐≤6 g/d、静坐时间<6 h/d、睡眠时间7~8 h/d、(18.5≤BMI<24)kg/m^(2)进行组合分析,研究健康生活方式与MS之间的关系。采用SPSS 23.0统计软件进行χ^(2)检验、Cox模型评估MS发病风险,以0~3种健康生活方式为参照组,计算各组风险比(HR)及95%CI值。结果单个健康生活方式与MS发病关联的分析结果发现,从不吸烟、从不饮酒、摄入低油低盐及正常BMI与MS发病的风险均呈负相关关系,未发现睡眠时间、静坐时间及蔬菜水果摄入与MS有关。但通过任意组合多种健康生活方式进行分析发现,在全人群中,与0~3种相比,保持5、6及7种以上健康生活方式的人群发生MS的风险分别为HR:0.760(95%CI:0.627~0.921)、HR:0.600(95%CI:0.485~0.742)及HR:0.541(95%CI:0.413~0.707)。BMI是影响MS的主要关联因素,其次是盐摄入及静坐时间。结论多种健康生活方式的协同作用能有效降低MS的发病风险,健康生活方式越多,风险越小。 Objective To investigate the impactof healthy lifestyle on the risk of metabolic syndrome(MS),and to provide reference for lifestyle intervention of MS.Methods Data were collected from the natural population cohort study of Guizhou Province.Among 9280 people in the baseline survey,3383 people who did not meet the inclusion criteria.After excluding thelost of follow-up and death,4663 participant were eventually included in the analysis.For 8 kinds of healthy lifestyle:never smoking,never drinking alcohol,vegetable and fruit intake≥400 g/d,edible oil intake≤30 g/d,salt intake≤6 g/d,sedentary behavior<6 h/d,sleep time 7-8 h/d,(18.5≤BMI<24)kg/m^(2),healthy lifestylecombinatorial analysis was performed to investigate the relationship between healthy lifestyle and MS.SPSS 23.0 was used to conduct Chi-square test and Cox model to evaluate the risk of MS.The risk ratio(HR)and 95%CI values of 0 to 3 healthy lifestyles were calculated as the reference group.Results The analysis of the association between a single healthy lifestyle and MS found that non-smoking,non-drinking,intake of low oil and salt,and normal BMI were negatively correlated with the risk of MS.No correlation was found between sleep time,sedentary behavior,and intake of vegetables and fruits withMS.But it was found through the analysis of any combination of multiple healthy lifestyles,in comparison to the 0-3,the risk of MS was HR:0.760(95%CI:0.627-0.921),HR:0.600(95%CI:0.485-0.742)and HR:0.541(95%CI:0.4313-0.707)for those who maintained 5,6 and 7 healthy lifestyles,respectively.BMI was the main correlation factor for MS,followed by salt intake and sitting time.Conclusion The synergistic effect of multiple healthy lifestyle can effectively reduce the risk of MS,and the more healthy lifestyle,the lower the risk.
