中国维持性血液透析患者低血压影响因素的meta分析

A meta-analysis of factors influencing hypotension in Chinese maintenance hemodialysis patients

目的系统分析维持性血液透析(maintenance hemodialysis,MHD)患者发生低血压的影响因素。方法计算机检索中国知网、万方、维普、PubMed、Embase、Cochrane Library、Web of Science、中国生物医学文献数据库(CBM)数据库中关于MHD患者低血压的研究报道,检索时间为建库至2023年6月,采用Revman软件进行meta分析。结果meta分析结果显示,年龄、心血管疾病、血红蛋白、白蛋白、超滤速度、血流量、透析年限、透析时间、超滤量/体质量、甲状旁腺激素、N末端B型钠尿肽前体(N-terminal B-type natriuretic peptide precursor,NT-proBNP)、血β2微球蛋白(β_(2) micro globulin,β_(2)-MG)、糖尿病、糖尿病肾病、营养不良、超滤率、透析频率、低密度脂蛋白-胆固醇与维持性血液透析患者发生低血压均有关。结论维持性血液透析患者发生低血压的影响因素众多,应尽早对维持性血液透析患者进行评估,对风险进行预防,避免低血压情况的发生。

Objective To systematically analyze the influencing factors of hypotension in maintenance hemodialysis patients.Methods The databases of CNKI,Wanfang,VIP,PubMed,Web of Science,Cochrane Library,CINAHL and Embase were searched for studies on hypotension in maintenance hemodialysis patients from the establishment of the database to June 2023.Revman software was used for meta-analysis.Results Meta-analysis results showed that,age,cardiovascular disease,hemoglobin,albumin,ultrafiltration rate,blood flow,dialysis years,dialysis time,ultrafiltration volume/body weight,parathyroid hormone,blood NT-proBNP,bloodβ2MG,diabetes,diabetic nephropathy,malnutrition,ultrafiltration rate,dialysis frequency and low density lipoprotein-cholesterol were associated with hypotension in maintenance hemodialysis patients Statistical significance.Conclusions There are many factors influencing the occurrence of hypotension in maintenance hemodialysis patients.It is necessary to evaluate and prevent the occurrence of hypotension in MHD patients as early as possible.
