摘要
目的分析无偿献血者核酸检测HBV DNA反应性样本的相关血清学标志物组合状态。方法选取邯郸市中心血站2018年1月-2019年2月留取的乙肝表面抗原(HBsAg)阴性、华益美核酸定性检测(nucleic acid testing,NAT)有反应性,经罗氏定性检测确认为反应性的41名献血者作为研究对象。使用化学发光定量测定其HBsAg、HBsAb、HBeAb、HBeAg和HBcAb,分析其免疫学指标与HBV DNA检测之间的关系。结果41份HBV DNA检测反应性样本,1份HBsAg化学发光检测阳性,HBeAg检测全部阴性,HBcAb阳性样本31份,其中HBcAb>7.0的样本数28份,CT值在35以上的低病毒载量样本26份。结论NAT法检测HBV可联合免疫学多指标评价输血传播残余风险,持续提升和评价核酸检测系统的灵敏性可提高临床输血的安全性。
Objective To analyze the combination status of serum markers related to nucleic acid testing of reactive HBV DNA samples of free donors.Methods The 41 blood donors with negative hepatitis B surface antigen(HBsAg),found to be reactive by nucleic acid testing(NAT)of Huayi from January 2018 to February 2019 at the central blood station in Handan City and confirmed to be reactive by Roche qualitative test were selected as study subjects.HBsAg,HBsAb,HBeAb,HBeAg and HBcAb of the subjects were quantitatively determined,the relations between immunological induices of such subjects and testing.of HBV DNA were analyzed.Results HBV DNA of 41 specimens were tested to be reactive,HBsAg of one case was tested to be positive by chemiluminescence method,HBeAg of all specimens was tested to be negative,the number of positive HBcAb cases was 31,the number of specimens with HBcAb>7.0 was 28,and the number of low viral load samples with CT values above 35 was 26.Conclusion NAT for testing HBV combined with multiple immunological indices is capable of evaluating the residual risk of blood transfusion,continuously enhancing and evaluating the sensitivity of nucleic acid testing system,and improving the safety of clinical blood transfusion.
