住院绝经期前女性患者骨折事件的病因分析

Analysis of the etiological factors of fracture events in hospitalized premenopausal female patients

目的探讨绝经期前女性或年轻女性骨折事件的病因。方法收集2010年1月-2020年1月某三甲医院收治的骨折患者,并分析其导致骨折的病因。结果366例女性年龄在20岁-44岁之间,平均年龄(35.46±6.62)岁,其中发生暴力性骨折的有169人,发生脆性骨折的有190人,其中186人为继发性骨质疏松导致的,导致继发性骨质疏松的病因包括类风湿关节炎、系统性红斑狼疮、多发性硬化症、强直性脊柱炎、肝病、乳糜泻、终末期肾病、维生素D缺乏、库欣综合症、甲状腺机能亢进、药物因素等,有3人是妊娠哺乳相关性骨质疏松导致的;发生病理性骨折的有7人。结论本研究显示绝经前或年轻女性骨折事件发生的主要病因是继发性骨质疏松,而绝经前或年轻女性的骨质疏松由于其发病率低很少受到关注,但在绝经前女性中继发性骨质疏松的人数不容小觑,应提高对绝经前女性骨质疏松的关注。

Objective To investigate the etiology of fracture events in premenopausal women or young women.Methods Patients with fracture admitted to a ClassⅢGrade A hospital from January 2010 to January 2020 were collected,and the causes of fracture were analyzed.Results Among the 366 women,the average age was(35.46±6.62)years(range 20-44 years).Among them,169 suffered from violent fracture and 190 suffered from fragility fracture,among which 186 were caused by secondary osteoporosis.The causes of secondary osteoporosis included rheumatoid arthritis,systemic lupus erythematosus,multiple sclerosis,celiac disease,ankylosing spondylitis,liver disease,end-stage renal disease,lack of vitamin D,Cushing's syndrome,hyperthyroidism,drug factors,and so on.There were 3 peoplepregnancy breast-feeding correlation relatedosteoporosis;Pathological fractures occurred in 7 persons.Conclusion This research shows that secondary osteoporosis is the main cause of fracture events in premenopausal or young women.Osteoporosis in premenopausal or young women due to its low incidence of little attention,but the number of secondary osteoporosis in premenopausal women to be reckoned with,should raise concern for osteoporosis in premenopausal women.
