球囊阻断泡沫硬化疗法治疗原发性下肢静脉曲张的疗效观察

Observation on the curative effect of balloon blocking foam sclerotherapy on primary varicose veins of lower limb

目的探讨球囊阻断泡沫硬化疗法治疗原发性下肢静脉曲张的疗效。方法收集2020年1月至2020年10月南阳市中心医院接受球囊阻断大隐静脉主干联合泡沫硬化疗法的30例原发性下肢静脉曲张患者的临床资料。所有患者分别于手术前后第1、3个月行静脉疾病严重程度评分(VCSS);进行血管超声检查,观察闭合情况、闭合残端距隐股交界(SFJ)长度及术后并发症情况。结果42条肢体全部完成球囊阻断大隐静脉主干联合泡沫硬化治疗;一期闭合率为95.24%,二期闭合率为100%;大隐静脉近端闭合处距离隐股交界处(1.68±0.08)cm。术后无深静脉血栓形成、肺栓塞等严重并发症发生,1条肢体轻微静脉炎经外用药物缓解。术后第1、3个月的VCSS评分(6.93±4.70)分和(3.00±0.78)分均低于术前(10.36±5.93)分,差异有统计学意义(P<0.05)。结论球囊阻断泡沫硬化疗法治疗原发性下肢静脉曲张疗效确切,可获得较高的短期闭合率。

Objective To investigate the effect of balloon blocking foam sclerotherapy on primary varicose veins of lower limbs.Method The clinical data of 30 patients with primary varicose veins of lower limbs who received balloon occlusion of the main trunk of the great saphenous vein combined with foam sclerotherapy in Nanyang Central Hospital from January 2020 to October 2020 were collected.All patients were evaluated by venous disease severity score(VCSS)before surgery,1 month after surgery,and 3 months after surgery;ultrasonography was performed to observe the closure of the vessels,the length of the closed stump from the SFJ and postoperative complications.Result Balloon occlusion of the main saphenous vein combined with foam sclerotherapy was completed in all 42 limbs;The primary closure rate was 95.24%,and the secondary closure rate was 100%;the proximal closure of the great saphenous vein was(1.68±0.08)cm from the saphenous femoral junction.There were no serious complications such as deep vein thrombosis and pulmonary embolism after operation,and one limb had slight phlebitis relieved by topical drugs.The VCSS(6.93±4.70)scores and(3.00±0.78)scores at 1 and 3 months after surgery were lower than those before surgery(10.36±5.93)scores,and the difference was statistically significant(P<0.05).Conclusion Balloon blocking foam sclerotherapy is effective in the treatment of primary varicose veins of lower extremities and can achieve high short-term closure rate.
