胃癌组织miR-148b和miR-1290及miR-141与胃癌患者Hp感染相关性探讨

Correlation of miR-148b, miR-1290 and miR-141 in gastric cancer tissues with Hp infection in gastric cancer patients

摘要 目的探讨幽门螺杆菌(Hp)感染与胃癌组织微小RNA(miR),miR-148b、miR-1290、miR-141的相关性,并分析各指标对肿瘤侵袭性的影响。方法收集2019-01-31-2020-01-31鞍山市中心医院收治的胃癌患者42例临床资料。采用荧光定量聚合酶链式反应(PCR)法检测Hp感染及胃癌组织miR-148b、miR-1290和miR-141表达情况,对比胃癌组织与癌旁正常组织miR-148b、miR-1290和miR-141表达,比较是否Hp感染患者临床资料及miR-148b、miR-1290和miR-141表达,logistic多因素回归模型分析胃癌患者Hp感染影响因素,ROC曲线分析miR-148b、miR-1290和miR-141表达对Hp感染的评估价值,Spearman相关系数模型分析各指标与临床病理特征的相关性,对比miR-148b、miR-1290、miR-141联合评估阳性与阴性患者预后。结果胃癌组织miR-148b表达(0.82±0.27)低于癌旁正常组织(1.25±0.31),差异有统计学意义(t=6.779,P<0.05),miR-141表达胃癌组织(1.27±0.32)低于癌旁正常组织(3.23±0.45),差异有统计学意义(t=19.426,P<0.05),miR-1290表达胃癌组织(4.72±0.98)高于癌旁正常组织(1.58±0.37),差异有统计学意义(t=23.004,P<0.05);胃癌Hp感染患者胃癌组织miR-1290表达(5.26±1.02)高于非Hp感染患者(3.75±0.74),差异有统计学意义(t=2.649,P<0.05),胃癌Hp感染患者胃癌组织miR-148b(0.75±0.19)表达低于非Hp感染患者(0.95±0.30),差异有统计学意义(t=5.033,P<0.05),胃癌Hp感染患者胃癌组织miR-141(1.06±0.25)表达低于非Hp感染患者(1.65±0.37),差异有统计学意义(t=6.157,P<0.05);临床分期(OR=4.607)、浸润深度(OR=5.325)、淋巴结转移(OR=3.468)、脉管瘤栓(OR=4.057)及胃癌组织miR-148b(OR=0.423)、miR-1290(OR=5.689)、miR-141(OR=0.405)表达均与胃癌Hp感染相关,均P<0.05;miR-148b、miR-1290、miR-141联合评估胃癌Hp感染的AUC为0.931,敏感度、特异度分别为85.19%、93.33%;胃癌Hp感染患者miR-148b、miR-1290、miR-141表达与临床分期(r值分别为-0.528、0.627、-0.682)、浸润深度(r值分别为0.615、0.631、-0.544)、淋巴结转移(r值分别为-0.576、0.624、-0.532)、脉管瘤栓(r值分别为-0.504、0.602、-0.567)间存在相关性,均P<0.05;miR-148b、miR-1290、miR-141联合评估阳性患者6个月病死率(4.55%)与阴性患者6个月病死率(0)比较,差异无统计学意义(χ^(2)=0.236,P=0.627),miR-148b、miR-1290、miR-141联合评估阳性患者12个月病死率(13.64%)与阴性患者12个月病死率(0)比较,差异无统计学意义,χ^(2)=0.767,P=0.381。结论胃癌Hp感染患者癌组织miR-148b、miR-141表达明显降低,miR-1290表达升高,各指标表达情况与Hp感染的发生密切相关,且与临床分期、浸润深度、淋巴结转移、脉管瘤栓密切相关,联合应用在评估胃癌Hp感染方面具有良好价值。 Objective To investigate the correlation between Helicobacter pylori(Hp)infection and gastric cancer tissue miR-148 b,miR-1290,miR-141,and to analyze the effect of each index on tumor aggressiveness.Methods Totally 42 patients with gastric cancer who were treated in our hospital from 2019-01-31 to 2020-01-31 were selected.Fluorescence quantitative PCR was used to detect Hp infection and miR-148 b,miR-1290 and miR-141 in gastric cancer tissue.The expression of miR-148 b,miR-1290 and miR-141 was compared between gastric cancer tissue and adjacent normal tissue,and the clinical data of patients with or without Hp infection were compared.logistic multivariate regression model was used to analyze the influencing factors of Hp infection in gastric cancer patients,and receiver operating characteristic(ROC)curve was used to analyze the evaluation value of miR-148 b,miR-1290 and miR-141 expression on Hp infection,Spearman’s correlation coefficient model was used to analyze the correlation between each index and clinicopathological characteristics,and to compare miR-148b,miR-1290,and miR-141to jointly assess the prognosis of positive and negative patients.Results The expression of miR-148bin gastric cancer tissues(0.82±0.27)was lower than that in normal tissues adjacent to cancer gastric cancer tissues(1.25±0.31),and the difference was statistically significant(t=6.779,P<0.05);the expression of miR-141(1.27±0.32)was lower than that in normal tissues adjacent to cancer(3.23±0.45),and the difference was statistically significant(t=19.426,P<0.05),miR-1290expression(4.72±0.98)was higher than that of normal tissues adjacent to cancer(1.58±0.37),and the difference was statistically significant(t=23.004,P<0.05);miR-1290expression in gastric cancer tissues of Hp-infected patients was(5.26±1.02)higher than that of non-Hp-infected patients(3.75±0.74),with a statistically significant difference(t=2.649,P<0.05);miR-148b(0.75±0.19)expression in gastric cancer tissues was lower than that in non-Hp-infected patients(0.95±0.30),with a statistically significant difference(t=5.033,P<0.05);miR-141(1.06±0.25)expression in gastric cancer tissues was lower than that in non-Hp-infected patients(1.65±0.37),with a statistically significant difference(t=6.157,P<0.05);clinical stage(OR=4.607),depth of infiltration(OR=5.325),lymph node metastasis(OR=3.468),vascular tumor thrombus(OR=4.057)and gastric cancer tissue miR-148b(OR=0.423),miR-1290(OR=5.689)and miR-141(OR=0.405)expression were significantly associated with Hpinfection in gastric cancer with statistically significant differences(P<0.05);the area under the curve(AUC)of miR-148b,miR-1290and miR-141combined to assess Hpinfection in gastric cancer was0.931,and the sensitivity and specificity were 85.19%,93.33%;the expressions of miR-148b,miR-1290,miR-141in patients with Hpinfected gastric cancer were associated with clinical stage(r=-0.528,0.627,-0.682),depth of infiltration(r=0.615,0.631,-0.544),lymph node metastasis(r=-0.576,0.624,-0.532),and vascular aneurysm emboli(r=-0.504,0.602,-0.567),P<0.05;the 6-month death rate in patients with positive miR-148b,miR-1290,and miR-141combined assessment(4.55%)compared with negative patients(0)was not statistically significant(χ^(2)=0.236,P=0.627),and the 12-month morbidity and mortality rate of patients with miR-148b,miR-1290,and miR-141combined assessment positive was(13.64%)compared with negative patients(0),and the difference was not statistically significant(χ^(2)=0.767,P=0.381).Conclusions The expression of miR-148band miR-141was significantly decreased and the expression of miR-1290was increased in patients with HP infection of gastric cancer The expression of each index was closely related to the occurrence of Hpinfection and closely correlated with clinical stage,depth of infiltration,lymph node metastasis and vascular tumor embolism.The combined application has good value in assessing Hpinfection in gastric cancer.
