摘要
目的:探究茯苓四逆汤化裁方对慢性充血性心力衰竭大鼠血浆BNP、ET、TNF-α水平及心脏病理组织学的影响,并探讨其治疗作用机制。方法:选取SD大鼠120只,随机抽取10只作为正常对照,其余采用盐酸阿霉素及氢化可的松腹腔注射,复制心肾阳虚型慢性充血性心力衰竭大鼠动物模型。造模成功后将大鼠随机分为6组:茯苓四逆汤化裁方高、中、低三个剂量组灌胃,阳性对照组用心宝丸灌胃,模型组、空白组给予等量蒸馏水。灌胃每天1次。用放射免疫分析法测定各组大鼠血浆BNP、ET、TNF-α的含量。计算各组大鼠心脏脏器系数,心脏标本用10%的甲醛溶液固定,送检,石钠常规包埋,HE染色,光镜下进行病理组织学检测。结果:模型组大鼠心肌细胞出现了不同程度肿胀、变性、坏死,并可见心肌细胞明显肥大、排列顺序紊乱。在治疗各组中,茯苓四逆汤化裁方低、高剂量组虽然普遍具有心肌细胞肥大现象,但与模型组比较,程度较轻,且无心肌灶性肿胀、变性、坏死及间质纤维化表现。茯苓四逆汤化裁方中剂组能显著降低模型大鼠血浆BNP、ET、TNF-α水平,与模型组比较有显著差异(P<0.05,P<0.01)。结论:茯苓四逆汤化裁方能通过降低血浆BNP、ET、TNF-α水平,有效地阻断CCHF模型大鼠心室重构效应。
Objective:Discussion on the effect of Modified Fuling Sini Recipe on BNP,ET,TNF-αlevel in plasma for rats with chronic heart failure and its influence on cardiology and histology and exploring its therapeutic action mechanisms.Methods:120 SD rats were selected and 10 rats were randomly selected as normal control,the other rats were given Doxorubicin hydrochloride and Hydrocortisone to establish rat model of chronic congestive heart failure(CCHF)with heart-kidney Yang-deficiency syndrome.After successful molding,rats were randomly divided into 6 groups:three groups of Modified Fuling Sini Recipe high,medium and low dose,the positive controlled groups was given Xinbao Pills with intragastric administration,and equal amounts of distilled water was given to the model and blank groups.The stomach was infused once a day.The plasma content of BNP,ET,TNF-αwere determined by radioimmunoassay.The cardiac organ coefficient was calculated in each group,and the heart samples were fixed with 10%formaldehyde solution for inspection,routinely embedded with stone sodium,stained with HE,and tested for pathological histology under light microscopy.Results:Rats cardiomyocytes of the model group showed different degrees of swelling,degeneration and necrosis,and showed obvious hypertrophy and disorder of cardiomyocytes.In each treatment group,although the low and high-dose groups generally had cardiomyocyte hypertrophy,compared with the model group,and there was no focal myocardial swelling,degeneration,necrosis and interstitial fibrosis.The medium group significantly reduced the plasma BNP,ET,and TNF-αlevels,as compared with the model group(P<0.05,P<0.01).Conclusion:It can effectively block the ventricular remodeling effect in CCHF model rats by reducing the plasma BNP,ET,and TNF-αlevels.
