升阳散火汤治疗糖尿病周围神经病变的疗效及对神经传导速度、炎症因子的影响

Efficacy of Using Shengyang Sanhuo Decoction in the Treatment of Diabetic Peripheral Neuropathy and Its Influence on Nerve Conduction Velocity and Inflammatory Factors

目的:探讨升阳散火汤治疗糖尿病周围神经病变的疗效及对神经传导速度、炎症因子的影响。方法:将2020年3月至2022年3月我院收治的146例糖尿病周围神经病变患者按照随机数字表法分为对照组和观察组各73例,对照组予甲钴胺治疗,观察组则联合升阳散火汤治疗,观察两组临床疗效、治疗前后两组中医证候积分、神经传导速度及炎症因子水平变化。结果:治疗3月后,观察组的临床有效率97.26%,高于对照组的87.67%(P<0.05);观察组治疗后中医证候积分低于对照组(P<0.05);观察组治疗后腓总神经及正中神经运动传导速度(MNCV)及感觉传导速度(SNCV)均高于对照组(P<0.05);观察组治疗后肿瘤坏死因子-α(TNF-α)、白介素6(IL-6)及C反应蛋白(CRP)水平均低于对照组(P<0.05)。结论:升阳散火汤治疗糖尿病周围神经病变疗效确切,能够提高周围神经传导速度,抑制炎性因子分泌。

Objective:To investigate the efficacy of using Shengyang Sanhuo decoction in treating diabetic peripheral neuropathy(DPN)and its influence on nerve conduction velocity and levels of inflammatory factors.Methods:146 patients with DPN who were treated in the hospital between March 2020 and March 2022 were classified into a controlled group(73 cases)and an observation group(73 cases)according to the random number table method.The controlled group was treated with Methylcobalamin while the observation group was combined with Shengyang Sanhuo Decoction.The clinical curative effect,as well as TCM syndromes scores,nerve conduction velocity and inflammatory factors levels before and after treatment were observed in the two groups.Results:After 3 months of treatment,the clinical effective rate of 97.26%in the observation group was higher than 87.67%in the controlled group(P<0.05).The scores of TCM syndromes in the observation group after treatment were lower than those in the controlled group(P<0.05).The motor nerve conduction velocities(MNCV)and sensory nerve conduction velocities(SNCV)of common peroneal nerve and median nerve were faster in the observation group than those in the controlled group after treatment(P<0.05).The levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)in the observation group after treatment were lower compared with those in the controlled group(P<0.05).Conclusion:Shengyang Sanhuo Decoction has a definite efficacy in the treatment of DPN,and it can improve the peripheral nerve conduction velocity and inhibit the secretions of inflammatory factors.
