山药化痰汤治疗对气阴两虚痰热壅肺型老年重症肺炎患者中医症状积分和肺功能的影响

Effect of Shanyao Huatan Decoction on Traditional Chinese Medicine Symptom Scores and Pulmonary Function for Elderly Patients with Severe Pneumonia Due to Qi-Yin Deficiency and Phlegm-heat Obstructing Lung

目的:分析山药化痰汤治疗对气阴两虚痰热壅肺型老年重症肺炎患者中医症状积分和肺功能的影响。方法:选取2020年10月~2021年10月在我院接受治疗的142例老年重症肺炎患者为研究对象。按随机数字表法分组:对照组(n=71)和观察组(n=71)。两组均接受西医基础治疗,观察组另加用山药化痰汤,对比两组中医症状积分、肺功能、疗效以及临床肺部感染评分(CPIS)与多器官功能障碍综合征(MODS)评分。结果:治疗后,两组各项中医症状积分及总分明显减少(P<0.05),观察组各项中医症状积分及总分明显低于对照组(P<0.05);观察组治疗有效率明显高于对照组(P<0.05);两组最大呼气流速峰值(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC指标明显升高(P<0.05),观察组PEF、FVC、FEV_(1)、FEV_(1)/FVC指标明显高于对照组(P<0.05);两组CPIS、MODS评分明显减少(P<0.05),观察组CPIS、MODS评分明显低于对照组(P<0.05)。结论:山药化痰汤可以进一步改善老年重症肺炎气阴两虚痰热壅肺型患者临床症状,促进其肺功能恢复。

Objective:To analyze the effect of Shanyao Huatan Decoction on TCM symptom scores and pulmonary function for elderly patients with severe pneumonia due to deficiency of Qi and Yin and phlegm-heat obstructing lung.Methods:A total of 142 elderly patients with severe pneumonia treated in the hospital between October 2020 and October 2021 were selected as the research subjects.They were divided into a controlled group and an observation group by the random number table method,with 71 cases in each group.Both groups were given basic western medicine treatment,and the observation group was additionally treated with Shanyao Huatan Decoction.The two groups were compared in terms of TCM symptom scores,pulmonary function,curative effects,the Clinical Pulmonary Infection Score(CPIS)and Multiple Organ Dysfunction Syndrome(MODS)scores.Results:After treatment,the scores of TCM symptoms and the total scores of the two groups were significantly reduced(P<0.05).The above scores of the observation group were significantly lower than those of the controlled group(P<0.05).The treatment response rate in the observation group was significantly higher than that in the controlled group(P<0.05).Peak expiratory flow(PEF),forced vital capacity(FVC),forced expiratory volume in 1 second(FEV_(1)),and FEV_(1)/FVC in the two groups were significantly increased(P<0.05),which were significantly higher in the observation group than in the controlled group(P<0.05).The CPIS and MODS scores of the two groups were significantly reduced(P<0.05),and the scores of the observation group were significantly lower than those of the controlled group(P<0.05).Conclusion:Shanyao Huatan Decoction can improve clinical symptoms and promote the recovery of pulmonary function for elderly patients with severe pneumonia due to deficiency of Qi and Yin and phlegm-heat obstructing lung.
