补肾生血方联合大剂量促红细胞生成素对老年血液透析患者肾性贫血的治疗效果研究

Curative Effect of Bushen Shengxue Recipe Combined with High-dose Erythropoietin on Renal Anemia for Elderly Hemodialysis Patients

目的:探讨补肾生血方联合大剂量促红细胞生成素对老年血液透析患者肾性贫血的治疗效果。方法:选取我院2017年7月~2020年7月76例老年血液透析肾性贫血患者,采用随机数字表法分为观察组(n=38)和对照组(n=38),对照组采用大剂量促红细胞生成素治疗,观察组在此基础上联合补肾生血方治疗。比较两组症状改善情况、治疗前后血常规变化、炎症因子水平、肾功能及不良反应发生情况。结果:两组临床治疗总有效率分别为94.74%、78.95%,观察组高于对照组(P<0.05);观察组治疗后食少纳呆、倦怠乏力、腰膝酸软、少气懒言等证候积分低于对照组(P<0.05);观察组治疗后Hct、Hb、RBC高于对照组(P<0.05);观察组治疗后CRP、TNF-α、IL-6水平低于对照组(P<0.05);观察组治疗后BUN、Scr水平低于对照组(P<0.05);观察组不良反应发生率为7.89%低于对照组的26.32%(P<0.05)。结论:补肾生血方联合大剂量促红细胞生成素能有效缓解老年血液透析患者肾性贫血的临床症状,改善血常规指标及肾功能,调节炎症因子水平,不良反应发生少,疗效确切。

Objective:To explore the curative effect of Bushen Shengxue Recipe combined with high-dose Erythropoietin on renal anemia for elderly hemodialysis patients.Methods:A total of 76 elderly patients with renal anemia undergoing hemodialysis in the hospital from July 2017 to July 2020 were enrolled and divided into an observation group(n=38)and a controlled group(n=38)by the random number table method.The controlled group was treated with high-dose Erythropoietin,while the observation group was additionally treated with Bushen Shengxue Recipe.The improvement of symptoms,changes of routine blood before and after treatment,levels of inflammatory factors,renal functionand occurrence of adverse reactions were compared between the two groups.Results:The total response rateof clinical treatment in the observation group was higher than that in the controlled group(94.74%vs 78.95%)(P<0.05).After treatment,scores of symptoms such as poor appetite,fatigue,soreness and weakness of waist and knees,and lack of energy in the observation group were lower than those in the controlled group(P<0.05),Hct,Hb and RBC were higher than those in the controlled group(P<0.05),levels of CRP,TNF-αand IL-6 were lower than those in the controlled group(P<0.05),BUN and Scr levels were lower than those in the controlled group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the controlled group(7.89%vs 26.32%)(P<0.05).Conclusion:Bushen Shengxue Recipe combined with high-dose Erythropoietin can effectively alleviate the clinical symptoms of renal anemia in elderly hemodialysis patients,improve blood routine indexes and renal function,and regulate levels of inflammatory factors,with few adverse reactions and significant curative effect.
