双黄液熏洗坐浴联合一次性根治术对低位肛周脓肿术后创面愈合的临床研究

Effect of Fumigation and Sitz Bath with Shuanghuang Liquid Combined with One-time Radical Operation on Postoperative Wound Healing of Low Perianal Abscess

目的:观察双黄液熏洗坐浴联合一次性根治术用于低位肛周脓肿对术后创面愈合的影响。方法:利用随机数表法将2019年1月-2021年1月本院肛肠科收治的98例低位肛周脓肿患者分为对照组与观察组,每组各49例,两组患者均接受一次性根治术治疗,术后对照组采用高锰酸钾溶液坐浴,观察组使用双黄液熏洗坐浴。比较两组治疗效果和术后恢复情况,并利用视觉模拟评分(VAS)及肛门失禁Wexner评分评估患者药浴治疗前后疼痛程度和肛门功能。结果:观察组的总有效率为93.88%,高于对照组的79.59%(P<0.05)。药浴治疗后,两组患者的VSA和Wexner评分均减少,且观察组不同时间点的VSA评分小于对照组(P<0.05),Wexner评分小于对照组(P<0.05)。治疗后,观察组的水肿消失、创面愈合及住院时间短于对照组(P<0.05)。结论:双黄液联合一次性根治术是低位肛周脓肿的有效治疗手段,并对术后创面愈合、肛门功能恢复及疼痛程度的减轻具有促进作用。

Objective:To study the effect of fumigation and sitz bath with Shuanghuang Liquid combined with one-time radical operation on postoperative wound healing of low perianal abscess.Methods:Using random number table method,98 patients with low perianal abscessadmitted to anorectal department of the hospital between January 2019 and January 2021 were divided into a controlled group and a study group,with 49 patients in each group.All patients enrolled were treated with one-timeradical operation.After operation,the controlled group received sitz bath with potassium permanganate solution,while the study group received fumigation and sitz bath with Shuanghuang Liquid.Therapeutic effects and postoperative recovery of the two groups were compared.The visual analogue scale(VAS)and the Wexner score were used to evaluate pain degree and anal function before and after medicated bath.Results:The total response rate in the study group was 93.88%,higher than 79.59%in the controlled group(P<0.05).After medicated bath,the VSA scores and Wexner scores of the two groups were reduced,and the above scores of the study group were lower than those of the controlled group(P<0.05).After treatment,the edema disappearance time,wound healing time and hospital stay of the study group were shorter than those of the controlled group(P<0.05).Conclusion:Fumigation and sitz bath with Shuanghuang Liquid combined with one-time radical operation is an effective treatment for low perianal abscess,and it can promote wound healing and recovery of anal function,and relieve pain after operation.
