血清单核细胞趋化蛋白-1水平在EV71型手足口病患儿中的变化及意义

Changes of serum monocyte chemoattractant protein-1 in hand-foot-and-mouth disease caused by EV71 infection

目的探讨血清单核细胞趋化蛋白-1(MCP-1)水平在肠道病毒71型(EV71)手足口病患儿中的变化及意义。方法收集2018年5月-2019年2月在河南省儿童医院诊治的120例由EV71病毒感染引起的手足口病患儿,其中轻症80例(轻症组),重症40例(重症组);同时选择健康儿童30名作为对照组,均抽取静脉血检测各组血清MCP-1、白细胞介素6(IL-6)和肿瘤坏死因子α(TNF-α)水平。结果重症组IL-6、TNF-α及MCP-1水平[(19.06±8.67)、(37.94±15.20)、(311.66±66.74)pg/mL]高于轻症组[(9.79±3.64)、(25.68±9.03)、(158.40±42.01)pg/mL]和对照组[(3.93±1.68)、(6.90±4.48)、(91.84±20.61 pg/mL)],差异均有统计学意义(P均<0.05);且轻症组IL-6、TNF-α及MCP-1水平高于对照组,差异均有统计学意义(P均<0.05)。手足口病患儿IL-6、TNF-α水平与MCP-1水平成正相关(r=0.711、0.679,P<0.05)。MCP-1水平与手足口病病情严重程度成正相关(r=0.815,P<0.05)。结论血清MCP-1水平与EV71病毒引起的手足口病的发生及病情严重程度相关,检测血清MCP-1水平有助于该疾病的早期诊断及病情评估。

Objective To investigate the level and significance of monocyte chemoattractant protein-1(MCP-1)in hand-foot-and-mouth disease caused by enterovirus 71(EV71)infection.Methods A total of 120 children with hand-foot-and-mouth disease caused by EV71 virus infection diagnosed and treated in Henan Children's Hospital from May 2018 to February 2019 were enrolled,including 80 mild cases(mild group)and 40 severe cases(severe group);30 healthy children were selected as control group.Venous blood was taken to examine MCP-1,interleukin 6(IL-6),tumor necrosis factor-α(TNF-α)levels.Results The levels of IL-6,TNF-αand MCP-1 in severe group[(19.06±8.67),(37.94±15.20),(311.66±66.74)pg/mL]were significantly higher than those in mild group[(9.79±3.64),(25.68±9.03),(158.40±42.01)pg/mL]and in control group[(3.93±1.68),(6.90±4.48),(91.84±20.61)pg/mL],the differences were statistically significant(P all<0.05);and the levels of IL-6,TNF-αand MCP-1 in mild group were significantly higher than those in control group,the differences were statistically significant(P all<0.05).The levels of IL-6 and TNF-αwere positively correlated with those of MCP-1(r=0.711,0.679;P all<0.05).The level of MCP-1 was positively correlated with the severity of hand-foot-and-mouth disease(r=0.815,P<0.05).Conclusions MCP-1 was associated with the severity of hand-foot-and-mouth disease.Detection of MCP-1 could be helpful for early diagnosis and severity of hand-foot-and-mouth disease.
