Surveillance of human brucellosis in Jiangmen, Guangdong, 2016-2018

摘要 目的了解广东省江门市人间布鲁氏菌病(布病)流行特征,为制定人间布病防治对策提供参考依据。方法对江门市人间布病病例流行病学特征及布病监测点监测结果进行描述性分析。采集监测点职业人群静脉血3~5 mL,分离血清行虎红平板凝集试验(RBPT)初筛和试管凝集试验(SAT)确认。结果2016-2018年江门市共报告人间布病病例113例,年均发病率为0.81/10万,无死亡病例,无暴发疫情。累计病例最多的3个区县分别是新会区(51例)、蓬江区(17例)和开平市(15例),3个区县病例数占全市总病例数的73.45%;113例布病病例中,男性占55.75%,女性占44.25%;各年龄段均有发病,50~59岁人群占总病例数的37.17%;职业分布中,农民占比最高,占总病例数28.32%;113例病例发病-诊断间隔中位数为13 d。108例个案调查病例的临床表现以发热(67.59%)、关节酸痛(46.30%)、乏力(43.52%)为主;48.15%有明确的动物接触史;32.74%是从事屠宰、饲养等的职业人群。2016-2018年职业人群血清学累计监测606人,累计阳性率为0.33%。结论持续开展布病监测可掌握疫情流行病学特征及其变化趋势;江门市布病流行水平逐年增高,需面向全人群广泛开展布病防控知识的健康教育,并采取有效措施提高医疗机构的布病诊疗能力。 Objecttive To understand the epidemiological characteristics of human brucellosis in Jiangmen city,Guangdong province,and provide scientific evidence for the prevention and control measures of brucellosis.Methods The descriptive epidemiological analysis was conducted on the epidemiological characteristics and surveillance results of human brucellosis in Jiangmen.Venous blood samples were collected from occupational population at monitoring points for 3 to 5 mL.Serum samples were isolated and confirmed by RBPT and SAT.Results A total of 113 brucellosis cases were reported with an average annual incidence of 0.81/100000 from 2016 to 2018,no death case and outbreaks were reported.The top 3 areas with more reported brucellosis cases were Xinhui district(51),Pengjiang district(17)and Kaiping district(15),and 73.45%of the total cases were reported in these 3 areas.Of all the reported cases,55.75%were male and44.25%were female.The incidence of the disease was found in all age groups,with 37.17%of the total cases aged 50~59 years.Among the occupational distribution,farmers(28.32%)accounted for the highest proportion.The median interval between onset and diagnosis was 39 days.The main clinical manifestations of 108 cases were fever(67.59%),arthralgia(46.30%)and fatigue(43.52%);48.15%had a clear history of animal contact;32.74%were occupational groups engaged in slaughtering and feeding.606 blood sample were collected from the occupational population and the cumulative positive rate was 0.33%.Conclusion Continuous surveillance of brucellosis could grasp epidemiological characteristics and trends of the epidemic situation.The epidemic level of brucellosis in Jiangmen city was increasing.Health education on brucellosis prevention and control should be widely carried out for the whole population,and effective measures should be taken to improve the diagnostic and therapeutic ability of medical institutions.
