胺碘酮致尖端扭转型室性心动过速1例报道

A Case Report of Torsades De Pointes Induced by Amiodarone

目的探讨胺碘酮导致尖端扭转型室性心动过速发生机理以及使用胺碘酮注意事项。方法收集1例胺碘酮导致尖端扭转型室性心动过速病例,回顾性分析心电图变化、治疗及转归,并复习相关文献。结果心电图确诊为尖端扭转型室性心动过速,停用胺碘酮及给予补钾补镁、异丙肾上腺素及利多卡因等药物治疗后,心电图改善。结论胺碘酮在治疗心律失常时有促心律失常作用,使用过程中,需密切监测电解质及心电图变化。

Objective To explore the mechanism of torsades de pointes induced by amiodarone and the attention in the use of amiodarone.Methods A case of torsades de pointes induced by amiodarone was collected.The ECG changes,treatment and outcome were analyzed retrospectively,and the related literature was reviewed.Results Torsades de pointes was diagnosed by the ECG.After stopping amiodarone and treatment with potassium and magnesium,isoproterenol and lidocaine,the ECG improved.Conclusion Amiodarone has the effect of promoting arrhythmia in the treatment of arrhythmia.It is necessary to closely monitor the changes of electrolyte and ECG in the process of use.
