温针灸联合中医定向透药对腰椎间盘突出患者IL-1α、IL-6、TNF水平及直腿抬高角度的影响

Treatment of IL-1 in Patients with Lumbar Intervertebral Disc Herniation by Warming Acupuncture and TCM Directional Transfusion α、Effect of IL-6,TNF levels and straight leg elevation angle

目的:探究温针灸联合中医定向透药对LDH患者IL-1α、IL-6、TNF水平及直腿抬高角度的影响。方法:以2018年7月—2019年6月我院收治的72例LDH患者作为研究对象,通过随机数表法分为联合治疗组、中医定向透药组,各36例。两组均予以西医基础治疗+中医定向透药,联合治疗组在此基础上予以温针灸治疗,两组均连续治疗10 d。观察并比较两组治疗前、治疗10 d后疼痛评分、炎性因子水平、直腿抬高角度及治疗10 d后临床疗效。结果:治疗10 d后,两组感觉PRI评分,感情PRI评分,VAS评分,血浆IL-1α、血清IL-6、血浆TNF水平,ODI,直腿抬高角度较治疗前降低,且联合治疗组低于中医定向透药组(P<0.05);两组直腿抬高角度较治疗前增加,且联合治疗组高于中医定向透药组(P<0.05)。联合治疗组的总有效率为94.44%(34/36),高于中医定向透药组的75.00%(27/36)(P<0.05)。结论:温针灸联合中医定向透药可有效抑制LDH患者炎性因子释放,减轻疼痛程度,改善直腿抬高角度,疗效显著,值得推广。

Objective:To explore the effects of warm acupuncture combined with traditional Chinese medicine directional penetration on the levels of IL-1,IL-6,TNF and the angle of straight leg elevation in patients with LDH.Methods:Taking 72 patients with LDH admitted to our hospital from July 2018 to June 2019 as the research object,they were randomly divided into the combined treatment group and the traditional Chinese medicine targeted penetration group,with 36 cases each.The two groups were treated with basic western medicine+targeted penetration of traditional Chinese medicine.The combined treatment group was treated with warm acupuncture on this basis.Both groups were treated continuously for 10 days.The pain score,inflammatory factor level,straight leg elevation angle and clini-cal efficacy of the two groups before and after 10 days of treatment were observed and compared.Results:After 10 days of treatment,the sensory PRI score,emotional PRI score,VAS score,plasma IL-1 score,serum IL-6,plasma TNF level,ODI,straight leg elevation angle of the two groups were lower than those before treatment,and the combined treatment group was lower than that of the traditional Chinese medicine directional penetration group(P<0.05);The straight leg elevation angle of the two groups increased compared with that before treatment,and that of the combined treatment group was higher than that of the traditional Chinese medicine directional penetration group(P<0.05).The total effective rate of the combined treatment group was 94.44%(34/36),which was higher than 75.00%(27/36)of the traditional Chinese medicine directional penetration group(P<0.05).Conclusion:Warm acupuncture and moxibustion combined with traditional Chinese medicine directional penetration can effectively inhibit the release of inflammatory factors in LDH patients,reduce the de-gree of pain,and improve the straight leg elevation angle.It has significant effect and is worth promoting.
