基于古籍文献挖掘探索肾性水肿朝代用药特点

Exploring the Characteristics of Dynasty Medication for Renal Edema Based on the Excavation of Ancient Literature

目的通过对肾性水肿古代文献的梳理,分析各代医家对肾性水肿的认识,考查其学术脉络及治疗规律,为现代临床用药及药物研发提供参考。方法通过"健康医疗大数据智能处理与知识服务系统"及课题组前期构建的中医药古籍全文文献库,对符合肾性水肿的相关条文进行病因病机及相关方剂方药组成的提取与梳理。结果分析结果发现每个朝代对于肾性水肿的治法及用药各有其特点,晋代偏于利水,汉唐两宋则注重补益,金元喜用理气,明清则在活血方面有更深一步的运用。治疗药物上,以茯苓、泽泻、甘草、白术、陈皮、木香居首。治法治则上,以阴阳虚实辨证为主,以发汗、利尿、峻下逐水为基本原则,以温补脾肾、补气养阴为本,以理气活血、利水通淋、清热解毒为标。结论中医古籍中对于肾性水肿的治疗方药存在着大量可供研究的数据,对古籍文献的梳理,能够让我们更好地挖掘和发挥中医药治疗肾性水肿的优势,对于临床方药的运用、药物的开发具有重要意义。

Objective Through combing the ancient literature of renal edema,this paper analyzes the understanding of renal edema among various generations of doctors,examines its academic context and treatment law.It can provides reference for modern clinical medication and drug development.Methods Through the“Health Care Big Data Intelligent Processing and Knowledge Service System”and the Chinese medicine ancient books full-text literature database constructed by the research group in the early stage,the etiology,pathogenesis and composition of relevant prescriptions and prescriptions were extracted and sorted out.Results The results showed that each dynasty had its own characteristics in the treatment and medication of renal edema.The Jin Dynasty was inclined to induce diuresis,the Han,Tang and Song Dynasties paid attention to tonifying,the Jin and Yuan Dynasties were fond of regulating Qi,and the Ming and Qing dynasties had a further application in promoting blood circulation.In the treatment of renal edema,Fuling(Poria),Zexie(Alismatis Rhizoma),Gancao(Glycyrrhizae Radix Et Rhizoma),Baizhu(Atractylodis Macrocephalae Rhizoma),Chenpi(Citri Reticulatae Pericarpium),Muxiang(Aucklandiae Radix)are the first.In terms of treatment principle,it is mainly based on syndrome differentiation of deficiency and excess of Yin and Yang,with the basic principles of sweating,diuresis,and severe water pursuit.Warming the spleen and kidney,tonifying Qi and nourishing Yin as the main,and regulating Qi and promoting blood circulation,promoting water circulation,clearing heat and detoxification as the standard.Conclusion There are a large number of data for research in the treatment of renal edema in ancient Chinese medicine books.Combing the ancient literature can help us to better explore and play the advantages of traditional Chinese medicine in the treatment of renal edema,which is of great significance for the application of Clinical prescriptions and drug development.
