摘要
目的系统分析挖掘唐汉钧教授治疗乳腺癌术后用药、药对及药症关系的用药规律,真实、客观地反映唐教授治疗乳腺癌术后的临床用药经验。方法将采集的病例数据标准化,建立Excel数据库,运用Netdraw软件,MATLAB编程,构建复杂网络,挖掘其用药、药对以及药症之间的关系。结果共收集乳腺癌术后女性患者201例,1299诊次病例。运用复杂网络分析方法,得出1299张处方206味药物中的排名前30位的核心药物:白术、红枣、丹参、白花蛇舌草、茯苓、淫羊藿、灵芝、莪术、黄芩、陈皮、党参、生黄芪、生甘草、紫苏梗、姜半夏、当归、蜂房、薏苡仁、炙黄芪、苍术、炙甘草、菊花、肉苁蓉、石见穿、五味子、干蟾皮、玄参、谷芽、麦芽、藿香。依据药对、药症配伍网络,结合唐汉钧教授本人意见,得出8个核心药对:灵芝伍淫羊藿;黄精伍山萸肉;玄参伍象贝;吴茱萸伍黄连;五味子伍酸枣仁;杜仲伍补骨脂;地锦草伍蚕茧;女贞子伍墨旱莲;并得出70组核心药症。结论唐汉钧教授治疗乳腺癌术后患者,采用扶正祛邪的治疗大法,扶正重在益气健脾、补养气血、补益肝肾、调和冲任。祛邪辨证选用清热化湿、祛瘀逐痰、解毒消肿、软坚散结诸法。临证辨病和辨证相结合,善用药对,随症加减,心药并举。
Objective To systematically analyze and explore drugs as well as the law of drug use in relation to drug and diseaseused by professor TANG Hanjun after the operation of breast cancer,and truly and objectively reflect professor TANG’sexperience on usingdrugs after the operation of breast cancer.Methods We standardized the collected case data,set up the EXCEL data base,used NETDRAW software and MATLAB programmingto construct a complex network and explored the relationship among drug use,drug pairs and drug-symptoms.Results There were 201 female patients after operation of breast cancer in total,1299 clinical cases.Using the complex network analysis method,we obtained the top 30 core drugs among the 206 drugs of 1299 prescriptions:Baizhu(RhizomaAtractylodisMacrocephalae),Hongzao(red dates),Danshen(Salvia miltiorrhizae),Baihuasheshecao(Hedyotisdiffusa),Fuling(Poriacoxula),Yinyanghuo(HerbaEpimedii),Lingzhi(GanodermaHerba),Ezhu(Zedoary),Huangqin(Radix Scutellariae),Chenpi(CitriReticulataePericarpium),Dangshen(Codonopsis Radix),Huangqi(Astragalus),Gancao(Glycyrrhizae Radix et Rhizoma),Zisugeng(Perilla stem),Banxia(Pinellia),Danggui(Angelica sinensis),Fengfang(hives),Yiyiren(kernel),processed Huangqin(sunburnt Astragalus root),Cangzhu(Atractylodes),processed Gancao(sunburnt Licorice root),Juhua(Chrysanthemum),Roucongrong(Cistanche),Shijianchuan(Shijianchao),Wuweizi(Schisandra chinensis),Ganchanpi(dried toads skin),Xuanshen(Radix Scrophulariae),Guya(Grainbud),Maiya(malt),Huoxiang(PogostemonisHerba).According to the drug matching network and professor TANG Hanjun’s own opinions,eight core drug pairs were obtained:Lingzhi(PogostemonisHerba)-Yinyanghuo(HerbaEpimedii),Huangjing(Polygonatum)-Shanzhuyu(Fructus Corni),Xuanshen(Radix Scrophulariae)-Xiangbeimu(Bulbus Fritillaria Thunbergii),Wuzhuyu(Evodia Rutaecarpa)-Huanglian(Coptischinensis),Wuweizi(Schisandra chinensis)-Suanzaoren(ZiziphiSpinosae Semen),Duzhong(Eucommia ulmoides)-Buguzhi(Fructus psoraleae),Dijincao(Humifuse euphorbia herb)-Canjian(Silkworm cocoon),Nuzhenzi(glossy privet fruit)-Mohanlian(Eclipta).And 70 groups of core drug-symptoms were obtained.Conclusion Professor TANG Hanjun adopts the method of promoting health and dispelling symptoms in treating patients with breast cancer after operation.Promoting health method focuses on tonifying Qi and spleen,nourishing Qi and blood,tonifying liver and kidney,harmonizing body.Removing pathogenic factors treatment uses removing heat and dampness,removing blood stasis and removing phlegm,detoxifying and reducing swelling,softening and dispelling the mass method.Professor TANG combines clinical syndrome differentiation and syndrome differentiation and uses drug pairs according to the symptoms.
