消肿膏外敷对复杂性胫骨平台骨折患者术后肿胀程度影响及康复作用研究

Study on Effect of Xiaozhong Ointment on Swelling Degree and Rehabilitation of Complex Tibial Platform Fractures after Operation

目的:探讨消肿膏外敷对复杂性胫骨平台骨折患者术后肿胀程度影响及康复作用。方法:将本院2010年4月—2017年9月复杂性胫骨平台骨折患者64例,随机分两组,两组均采用双钢板内固定术治疗,对照组术前采用冰袋持续冷敷48 h;观察组术前采用消肿膏外敷,隔日1次,持续治疗7 d。比较两组患者临床观察指标、视觉模拟量表评分(visual analogue scale,VAS)、肿胀度评分及胫骨平台内翻角(tibial plateau varus angle,PT)、后倾角(tibial plateau obliquity,PSA)、Lysholm评分,记录术后并发症状况。结果:对照组有效率75.00%低于观察组93.75%,差异具有统计学意义(P<0.05)。对照组在骨折愈合时间、术后负重时间[(5.19±1.56)个月、(41.36±6.57)d]上高于观察组[(4.41±1.23)个月、(36.85±6.12)d],差异具有统计学意义(P<0.05)。对照组和观察组术后3 d VAS评分为1.43±0.56、1.04±0.49,肿胀度评分为1.41±0.47、1.05±0.42,术后7 d VAS评分为0.90±0.43、0.57±0.39,肿胀度评分为0.93±0.42、0.59±0.38,术后7 d Lysholm评分为68.24±8.36、74.24±7.49,术后3个月Lysholm评分为79.21±8.67、85.91±9.31,对照组和观察组上述指标比较,差异具有统计学意义(P<0.05)。两组患者未出现固定物松动、脱落、折断等不良事件。结论:消肿膏外敷对复杂性胫骨平台骨折患者的疗效确切,可短时间内缓解肿胀、疼痛,促进膝关节功能康复,值得推广。

Objective:To investigate the effect of Xiaozhong Ointment on swelling degree and rehabilitation of complex tibial platform fractures after operation.Methods:Sity-four patients with complex tibial plateau fractures in our hospital from April 2010 to September 2017 were randomly divided into two groups.Both groups were treated with double plate internal fixation.The control group was treated with ice bag continuous compressing for 48 hours before operation.The observation group was treated with Xiaozhong Ointment by external application before operation,once every other day for 7 days.The clinical observation index,visual analogue scale(VAS),swelling score,tibial plateau varus angle(PT),tibial plateau obliquity(PSA)and Lysholm score were compared between the two groups,and the complications were recorded.Results:The effective rate of the control group(75.00%)was lower than that of the observation group(93.75%)(P<0.05).The healing time of fracture and the weight-bearing time after operation in the control group were(5.19±1.56)month and(41.36±6.57)d,which were higher than the observation group’s(4.41±1.23)month and(36.85±6.12)d and the difference was statistically significant(P<0.05).Three days after treatment,VAS scores in the control group and observation group were 1.43±0.56 and 1.04±0.49,swelling scores were 1.41±0.47 and 1.05±0.42.VAS scores 7 days after operation were 0.90±0.43 and 0.57±0.39,swelling scores were 0.93±0.42 and 0.59±0.38,Lysholm scores 7 days after operation were 68.24±8.36 and 74.24±7.49,Lysholm scores 3 days after operation were 79.21±8.67 and 85.91±9.31 and the differences were statistically significant(P<0.05).There were no adverse events such as loosening,falling off or breaking of fixtures in the two groups.Conclusion:External application of Xiaozhong Ointment has definite curative effect on complex tibial plateau fracture patients.It can relieve swelling and pain in a short time and promote the rehabilitation of knee joint function.It is worthy of popularizing.
