灵芝胶囊联合同步放化疗对宫颈癌并发人乳头瘤病毒感染患者免疫功能的影响

Effect of Ganoderma lucidum capsule combined with concurrent chemoradiotherapy on level of immune function in patients with cervical cancer complicated with human papillomavirus infection

目的探讨灵芝胶囊联合同步放化疗对宫颈癌并发人乳头瘤病毒感染患者免疫功能的影响,为患者临床治疗提供依据。方法选取河南省直第三人民医院2018年1月—2020年1月收治的宫颈癌并发HPV感染患者150例,随机分为观察组和对照组各75例。对照组患者给予同步放化疗,化疗采用顺铂(30 mg/m^(2))和紫杉醇(60 mg/m^(2))进行治疗,放疗采用腔内后装放射治疗和调强放疗。观察组在对照组的基础上给予灵芝胶囊。治疗后,对两组患者临床效果、外周血T淋巴细胞亚群、血清细胞因子水平及血管内皮细胞生长因子(Vascular endothelial cell growth factor,VEGF)和CD105蛋白进行比较。结果治疗后总缓解率观察组为86.67%,对照组为72.00%,组间对比差异有统计学意义(P<0.05);观察组治疗后CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)分别为(39.23±4.45)%、(25.24±3.21)%、(1.49±0.18),对照组CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)分别为(31.23±3.83)%、(30.34±4.01)%、(1.19±0.16),两组患者CD4^(+)、CD4^(+)/CD8^(+)均升高,观察组上升程度与对照组比较差异有统计学意义(P<0.05),治疗后,两组患者CD8^(+)均下降,观察组下降程度较对照组显著(P<0.05);两组治疗前IL-6、TNF-α、CRP水平差异无统计学意义(P>0.05),治疗后两组IL-6、TNF-α、CRP水平均下降,观察组较对照组低(P<0.05);两组患者治疗前VEGF、CD105水平差异无统计学意义(P>0.05),治疗后两组患者VEGF、CD105水平均降低,观察组低于对照组,对比具有统计学意义(P<0.05)。结论灵芝胶囊联合同步放化疗可有效改善宫颈癌并发人乳头瘤病毒感染患者治疗效果,提高患者免疫功能水平,抑制肿瘤新生血管的形成。

Objective To explore the effect of Ganoderma lucidum capsule combined synchronous radiotherapy and chemotherapy with cervical cancer complicated with human papilloma virus infection,and to provide the basis for clinical treatment.Methods Totally 150 cases of cervical cancer complicated with HPV infection in our hospital from Jan.2018 to Jan.2020 were selected.According to the random number table method,the patients were randomly divided into observation group with 75 cases and control group with 75 cases.Control group received concurrent chemoradiotherapy,chemotherapy with cisplatin 30 mg/m^(2)and paclitaxel 60 mg/m^(2),and radiotherapy with intravitary retroloading and intensemodulated radiotherapy,and the observation group was given Lingzhi Capsule on the basis of the control group.After treatment,the clinical effects,peripheral blood T lymphocyte subsets,serum cytokine levels and VEGF,CD105 were compared between the two groups.Results The total remission rate after treatment was 86.67%in the observation group and 72.00%in the control group,and the difference between groups was statistically significant(P<0.05).CD4^(+),CD8^(+),CD4^(+)/CD8^(+)in the observation group were(39.23±4.45)%,(25.24±3.21)%,(1.49±0.18),respectively.CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)in the control group were(31.23±3.83)%,(30.34±4.01)%and(1.19±0.16),respectively.CD4^(+)and CD4^(+)/CD8^(+)in the two groups were all increased,and the degree of increase in the observation group was statistically significant compared with that in the control group(P<0.05).After treatment,CD8^(+)decreased in both groups,and the degree of decrease in observation group was significantly higher than that in control group(P<0.05).There were no significant differences in the levels of IL-6,TNF-αand CRP between 2 groups before treatment(P>0.05).After treatment,the levels of IL-6,TNF-αand CRP in 2 groups were decreased,and the observation group was lower than the control group(P<0.05).There were no significant differences in the levels of VEGF and CD105 in 2 groups before treatment(P>0.05).After treatment,the levels of VEGF and CD105 in 2 groups were decreased,and the observation group was lower than the control group,and the comparison was statistically significant(P<0.05).Conclusion Ganoderma lucidum capsule combined with concurrent chemoradiotherapy can effectively improve the treatment effect of patients with cervical cancer complicated with human papillomavirus infection,improve the level of immune function of patients,inhibit the formation of tumor neovascularization.
