小切口非超声乳化白内障手术治疗硬核白内障的临床效果研究

Clinical effect of small incision non-phacoemulsification cataract surgery for hard nucleus cataract

目的研究小切口非超声乳化白内障手术治疗硬核白内障的效果及在临床中的优势。方法回顾性研究2019年1月~2021年1月福建医科大学附属宁德市医院进行治疗的硬核白内障患者130例,按照随机化的原则分为对照组及试验组,每组65例。对照组患者采用传统超声乳化白内障手术进行治疗,试验组患者使用小切口非超声乳化白内障手术进行治疗,在手术后对其随访1~3个月。观察术前以及术后视力、眼压、手术时间、并发症、房水细胞分级和眼表情况。结果与对照组比较,试验组患者在术后7天及术后15天时视力提高(P<0.05);手术所需时间试验组明显短于对照组(P<0.01);试验组手术后并发症发生率明显降低(P<0.05),术后7天、15天房水细胞分级显著低于对照组(P<0.05),并且试验组术后早期泪液分泌试验(Schirmer’s test,ST)及泪膜破裂时间(Break up time,BUT)均高于对照组(P<0.05)。与对照组比较,试验组术后眼压无显著差异(P>0.05)。结论小切口非超声乳化白内障手术治疗硬核白内障可以有效提升手术后视力,减少手术时间,降低并发症的出现,安全程度有所保障,在临床中应用价值较高,可推广使用。

Objective To study the effect of small incision non-phacoemulsification cataract surgery in the treatment of hard nucleus cataract and its clinical advantages.Methods A retrospective study was conducted on 130 patients with hard nucleus cataract who were treated in Ningde Hospital Affiliated to Fujian Medical University from January 2019 to January 2021.They were randomly divided into control group and test group,with 65 cases in each group.The patients in the control group were treated with traditional phacoemulsification cataract surgery,while the patients in the test group were treated with small incision non-phacoemulsification cataract surgery,and the patients were followed up for 1~3 months after surgery.The visual acuity,intraocular pressure,operation time,complications,aqueous humor cell grade and ocular surface condition before and after operation were observed.Results Compared with the control group,the visual acuity of the test group was significantly improved at 7 days and 15 days after operation(P<0.05),the operation time was significantly shorter(P<0.01),and the incidence of postoperative complications was significantly reduced.The aqueous humor cells grade at 1 days and 15 days after operation were significantly lower than those in the control group(P<0.05),and the Schirmer’s test(ST)and break up time(BUT)in the test group were higher than those in the control group(P<0.05).Compared with the control group,there was no significant difference in postoperative intraocular pressure in the test group(P>0.05).Conclusion Small incision non-phacoemulsification cataract surgery can effectively improve postoperative vision,reduce the operation time,reduce the occurrence of complications,and ensure the degree of safety in the treatment of hard nucleus cataract.It has high value in clinical application and can be widely used.
