鼻内窥镜下手术与传统手术治疗慢性鼻窦炎鼻息肉的疗效对比分析

Comparative analysis of the curative effect of nasal endoscopic surgery and traditional surgery on chronic sinusitis and nasal polyps

目的:探讨鼻内窥镜下手术与传统手术治疗慢性鼻窦炎鼻息肉的疗效。方法:收集2011年1月~2016年12月入院的120例慢性鼻窦炎鼻息肉患者随机分为两组,鼻内窥镜组患者给予鼻内窥镜下手术,传统组患者则给予传统手术治疗,比较两组患者围手术期指标、并发症发生率、嗅觉功能、相关因子水平、总体治疗效果与生活质量。结果:鼻内窥镜组患者手术时间(45.60±7.89)、术腔评分(6.12±0.75)与住院时间(3.17±0.66)均显著性低于传统组(45.60±7.89,107.89±35.90,7.42±0.83,5.24±0.91);治疗后VAS评分(1.15±0.40)与ECP(3.12±0.63)阳性细胞计数显著性低于传统组(1.66±0.48,3.80±0.61);治疗后1年SNOT-20总分(4.79±1.05)组间比较显著性低于传统组(7.09±1.13),存在显著性差异;总体并发症发生率(13.33%)明显低于传统组(28.33%);总有效率(95%)明显高于传统组(81.67%),存在明显差异。结论:慢性鼻窦炎鼻息肉应用鼻内窥镜下手术的治疗效果较传统手术治疗效果更好,安全性更高,具有临床借鉴意义。

Objective To explore the effect of nasal endoscopic surgery and traditional surgery on chronic sinusitis and nasal polyps.Methods 120 patients with chronic sinusitis and nasal polyps admitted to hospital from January 2011 to December 2016 were randomly divided into two groups.Patients in the nasal endoscopy group underwent nasal endoscopic surgery and patients in the traditional group received conventional surgery The perioperative indexes,complication rates,olfactory function,related factors,overall therapeutic effect and quality of life were compared between the two groups.Results The operating time(45.60±7.89),cavity score(6.12±0.75)and length of stay(3.17±0.66)were significantly lower in the nasal endoscopic group than in the traditional group(45.60±7.89,107.89±35.90,7.42±0.83,5.24±0.91);The VAS scores(1.15±0.40)and ECP(3.12±0.63)positive cell counts after treatment were significantly lower than those in the traditional group(1.66±0.48,3.80±0.61);total 1 year SNOT-20 after treatment The scores in the subgroups(4.79±1.05)were significantly lower than those in the traditional group(7.09±1.13),with significant differences.The overall complication rate(13.33%)was significantly lower than the traditional group(28.33%).The total effective rate(95%)was significantly higher than that of the traditional group(81.67%),and there was a significant difference.Conclusion Compared with traditional surgery,the treatment of chronic sinusitis and nasal polyps under endoscopic sinus surgery is more effective,safer and better prognosis.
