沐舒坦预防老年脑出血患者早期ARDS效果分析及对肺功能的影响

Effects of Mucosolvanon preventing early ARDS and its influences on lung function in elderly patients with cerebral hemorrhage

目的:探讨沐舒坦预防老年脑出血患者早期急性呼吸窘迫综合征(ARDS)的效果及对肺功能的影响。方法:选取2014年2月~2019年3月收治于我院的102例老年脑出血患者作为研究对象,随机分配为观察组(51例)和对照组(51例)。对照组采用常规治疗方案,在对照组基础上采用沐舒坦治疗,比较两组患者治疗后的ARDS发生率,治疗前后氧合指数(PaO2/FiO2)、二氧化碳分压(PaCO2)、氧分压(PaO2),血清白细胞介素-6(IL-6)、C反应蛋白(CRP)和肿瘤坏死因子-α(TNF-α)水平,第一秒用力呼气量(FEV1)、用力肺活量(FVC)、最大呼气峰流速值(PEF),治疗后7d的APACHE-II评分、SOFA评分,以及治疗期间不良反应发生率。结果:经治疗后,观察组患者的PaO2、PaCO2、PaO2/FiO2、FEV1、FVC和PEF均高于对照组,血清IL-6、CRP、TNF-α水平ARDS发生率明显低于对照组;治疗7d后观察组患者APACHE-II评分和SOFA评分低于对照组;两组治疗期间的不良反应发生率无明显差异。结论:沐舒坦用于老年脑出血患者可减轻机体炎性反应、改善血液氧合情况和肺功能,预防早期ARDS的发生,并促进病情恢复。

Objective To explore effects of Mucosolvanon preventing early acute respiratory distress syndrome(ARDS)and its influences on lung function in elderly patients with cerebral hemorrhage.Methods A total of 102 elderly patients with cerebral hemorrhage who were admitted to the hospital from February 2014 to March 2019 were enrolled asstudy objects.They were randomly divided in to observation group(51cases)and control group(51cases).The control group was treated with routine treatment regimen,while observation group was treated with Mucosolvanonbasis of control group.The incidence of ARDS after treatment,oxygen ationindex(PaO2/FiO2),partial pressure of carbondioxide(PaCO2),oxygen partial pressure(PaO2),level so fserum interleukin-6(IL-6),C-reactionprotein(CRP)and tumor necrosis factor-α(TNF-α),forced expiratory volume in 1second(FEV1),forced vital capacity(FVC)and peak expiratory flow(PEF)before and after treatment,APACHE-II score and SOFA score of treatment,and incidence of adverse reactions during treatment were compared between the two groups.Results After treatment,PaO2,PaCO2,PaO2/FiO2,FEV1,FVC and PEF in observation group were higher than thosein control group,while levels of serum IL-6,CRP and TNF-α,and incidence of ARDS were significantly lower than those in control group.After 28 d of treatment,scores of APACHE-II and SOFA in observation group were lower than those in control group.There was no significant difference in incidence of adverse reactions during treatment between the two groups.Conclusion The application of Mucosolvanin elderly patients with cerebral hemorrhage can alleviate inflammatory response,improve blood oxygenation and lung function,prevent occurrence of early ARDS,and promote there covery of the disease.
