摘要
目的:分析视网膜光凝术对增殖前期及增殖期糖尿病视网膜病变的治疗效果。方法:对我院2018年1月—2019年6月收治的糖尿病视网膜病变患者进行回顾式分析,79例(104眼)增殖期组对象随机抽取于增殖期糖尿病视网膜病变患者,165例(198眼)增殖前期患者以同样方式抽取于增殖前期糖尿病视网膜病变患者,所有患者均接受视网膜光凝术治疗,观察两组治疗后视力变化情况、眼底变化及视网膜病变变化,并统计并发症发生情况。结果:治疗后,增殖前期组视力改善优于增殖期组;增殖前期组眼底改善总有效率92.42%及视网膜病变改善有效率96.97%,均显著高于增殖期组的79.81%、87.50%;增殖前期组的并发症发生率6.06%,显著低于增殖期组13.46%,差异存在统计学意义(P<0.05)。结论:在糖尿病视网膜病变的增殖前期积极接受视网膜光凝术治疗对视力改善有明显的促进作用,且患者越早进行激光治疗,预后越好。
Objective:To analyze the therapeutic effect of retinal laser photocoagulation for treatment of preproliferative and proliferative diabetic retinopathy(DR).Methods:A retrospective analysis was performed on DR patients admitted to our hospital between January 2018 and June 2019.From patients with proliferative DR,79(104 eyes)were randomly selected and assigned to the proliferative DR group;from patients with preproliferative DR,165(198 eyes)were randomly selected and assigned to the pre-proliferative DR group.All patients underwent retinal laser photocoagulation.The changes in visual acuity,fundus and retinal lesions,as well as complications,were recorded and compared between the two groups.Results:After treatment,the visual acuity improvement in the pre-proliferative DR group was better than that in the proliferative DR group.More patients in the pre-proliferative DR group experienced improvements in the fundus(92.42%vs 79.81%)and retinal lesions(96.67%vs 87.50%),compared with the proliferative DR group.The rate of complication in the pre-proliferative DR group was 6.06%,which was lower than that in the proliferative DR group(13.46%),with statistically significant difference(P<0.05).Conclusion:Aggressive use of retinal laser photocoagulation during the preproliferative phase of DR may result in significant improvement in visual acuity.The treatment outcome can be better with earlier use of laser therapy.
