肛瘘的高分辨率MRI特征与其中医辨证分型的相关性研究

High-resolution MRI Features of Anal Fistula,and TCM Differentiated Typing on it:A Relativity Study

目的:通过分析不同中医辨证分型肛瘘的高分辨率磁共振(MRI)特征,探讨高分辨率MRI特征与肛瘘中医辨证分型的相关性。方法:以2021年3月至2022年2月于许昌中医院就诊并行高分辨率MRI检查的67例肛瘘患者为观察对象。根据患者中医辨证分型分组,湿热下注型组(44例)、正虚邪恋型组(16例)和阴液亏虚型组(7例)。分析各组高分辨率MRI检查所示内口位置、瘘管数目、瘘管深度、瘘管长度、有无脓肿、表观弥散系数(ADC)值等情况。结果:3组肛瘘患者内口位置、瘘管数量、瘘管深度、瘘管长度、有无脓肿方面比较差异无统计学意义,P>0.05。阴液亏虚型组ADC值明显高于湿热下注型组、正虚邪恋型组,P<0.05;但湿热下注型组、正虚邪恋型组ADC值比较差异无统计学意义,P>0.05。结论:高分辨率MRI特征与肛瘘中医辨证分型有一定相关性,ADC值或可为肛瘘中医分型提供参考。

Objective Through analysing the high-resolution MRI features of anal fistula with different TCM differentiated types,to explore the relativity of both.Methods Enrolled 67 anal fistula patients treated and inspected with high-resolution MRI in author’s hospital(2021-03-2022-02)into this study:they were typed and grouped as 44 cases being dampness-heat in lower jiao-type(group),16 cases being lingering pathogen due to deficient vital qi-type(group),and 7 cases being Yin deficiency-type(group);then,also their MRI shown inner-orifice site;fistula(syrinx)numbers,length,and depth,as well as with or without abscess,ADC values,etc were analysed.Results In inner-orifice site,as well as syrinx’numbers,length,depth,and with or without abscess there was no statistical difference between the three groups(P>0.05);only in ADC values Yin deficiency-type(group)was significantly higher than other two types(groups)(P<0.05),but between later two types(groups)there was no statistical difference in ADC values(P>0.05).Conclusion Between the MRI feature and TCM differentiated types of anal fistula there is certain relativity,and ADC values might be a refference for TCM typing.
