改良阴道残端缝合对全子宫切除术患者盆底功能、性功能及并发症的影响

Effects of Modified Vaginal Stump Suture on Pelvic Floor Function,Sexual Function and Complications in Patients with Total Hysterectomy

目的:分析改良阴道残端缝合对全子宫切除术患者盆底功能、性功能及并发症的影响.方法:选取本院2019 年 3 月至 2022 年 3 月期间收治的 101 例行全子宫切除术的患者作为研究对象,根据阴道缝合方法不同分为观察组(n =52,改良阴道残端缝合术)和对照组(n =49,连续缝合).对比两组患者的盆底功能、性生活及并发症发生情况.结果:与手术前相比,两组患者术后的盆底功能障碍性疾病问卷(PFDI)-20、盆底疾病生活质量影响问卷(PFIQ)-7 评分均有所下降,其中观察组下降更为显著(P<0.05);观察组患者性生活无变化发生率(84.62%,44/52)高于对照组(65.31%,32/49),观察组性功能减弱发生率(7.69%,4/52)低于对照组(32.65%,16/49)(P<0.05);观察组的并发症发生率明显低于对照组(P<0.05).结论:改良阴道残端缝合对全子宫切除患者盆底功能、性功能的影响较小,且术后并发症发生率较低,有效缩短患者恢复的时间,值得在临床推广.

Objective:To analyze the effects of modified vaginal stump suture on pelvic floor function,sexual function and complications in patients with total hysterectomy.Methods:A total of 101 patients who underwent total hysterectomy received in our hospital from March 2019 to March 2022 were selected as the research subjects,and according to different suture methods,they were divided into observation group(n=52,modified vaginal stump suture)and control group(n=49,continuous suture).The pelvic floor function,sexual life and complications were compared between the two groups.Results:Compared with before operation,the pelvic floor distress inventory-short form(PFDI)-20 and pelvic floor impact questionnaire(PFIQ)-7 scores of the two groups were decreased after operation,among which the observation group was more significant(P<0.05);the incidence of no change in sexual life in the observation group(84.62%,44/52)was higher than that in the control group(65.31%,32/49),and the incidence of sexual dysfunction in the observation group(7.69%,4/52)was lower than that in the control group(32.65%,16/49)(P<0.05);the incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:The modified vaginal stump suture has little effect on pelvic floor function and sexual function in patients with total hysterectomy,and the incidence of postoperative complications is low,which can effectively shorten the recovery time of patients,and is worthy of clinical promotion.
