肌电生物反馈疗法联合康复训练对脑卒中后偏瘫患者康复效果的影响分析 被引量:2

摘要 目的分析肌电生物反馈疗法联合康复训练在脑卒中后偏瘫患者中的临床应用效果.方法选取该院2021年1月—2022年1月收治的226例脑卒中后偏瘫患者为研究对象,根据随机数字表法分为两组,每组113例.对照组采用常规康复训练,观察组在对照组基础上联合肌电生物反馈疗法.分析并对比两组干预效果.结果干预前,两组各方向肩关节活动度比较,组间差异无统计学意义(P>0.05);干预后,两组各方向肩关节活动度均较同组干预前增大,且观察组肩关节外展、外旋、内旋活动度均大于对照组,组间差异有统计学意义(P<0.05).干预前,两组上肢、下肢运动功能评分比较,组间差异无统计学意义(P>0.05);干预后,两组上肢、下肢运动功能评分均较同组干预前升高,且观察组上肢、下肢运动功能评分均高于对照组,组间差异有统计学意义(P<0.05).干预前,两组改良Barthel指数(MBI)评分比较,差异无统计学意义(P>0.05);干预后,两组MBI评分均高于同组干预前,且观察组MBI评分高于对照组,组间差异有统计学意义(P<0.05).干预前,两组腕伸肌、胫前肌积分肌电值(iEMG)比较,组间差异无统计学意义(P>0.05);干预后,两组腕伸肌、胫前肌iEMG均较同组干预前升高,且观察组腕伸肌、胫前肌iEMG均高于对照组,组间差异有统计学意义(P<0.05).结论脑卒中后偏瘫患者采用肌电生物反馈疗法联合康复训练可增加患者的肩关节活动度,改善肢体运动功能,促进日常生活能力提高. Objective To analyze the clinical application effect of electromyographic biofeedback therapy combined with rehabilitation training in patients with hemiplegia after stroke.Methods 226 patients with hemiplegia after stroke admitted to the hospital from January 2021 to January 2022 were selected as the study objects,and were divided into two groups according to random number table method,with 113 cases in each group.The control group received routine rehabilitation training,and the observation group was combined with electromyographic biofeedback therapy on the basis of control group.The intervention effects of the two groups were analyzed and compared.Results Before intervention,there was no significant difference in the range of motion of shoulder joint in all directions between the two groups(P>0.05);after intervention,the range of motion of shoulder joint in all directions in both groups increased compared with those of the same group before intervention,and the abductor,external rotation and internal rotation of shoulder joint in the observation group were greater than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Before interven tion,there was no significant difference in motor function scores of upper and lower limbs between the two groups(P>0.05);after intervention,the motor function scores of upper and lower limbs in both groups were higher than those of the same group beSWFKfore intervention,and the motor function scores of upper and lower limbs in the observation group were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Before intervention,there was no significant difference in modified Barthel index(MBI)score between the two groups(P>0.05);after intervention,the MBI scores in both groups were higher than those of the same group before intervention,and the MBI score of the observation group was higher than that of the control group,and the differences between the two groups were statistically significant(P<0.05).Before intervention,the integrated electromyogram(iEMG)of the wrist extensor muscle and anterior tibial muscles were compared between the two groups,and the differences between the groups were not statistically significant(P>0.05);after intervention,iEMG of wrist extensor muscle and anterior tibial muscles in both groups increased compared with those of the same group before intervention,the iEMG of wrist extensor muscle and anterior tibial muscles in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The use of electromyographic biofeedback therapy combined with rehabilitation training in patients with hemiplegia after stroke can increase the patients'shoulder joint range of motion,improve the motor function of the limbs,and promote the improvement of the ability of daily living.
