左卡尼汀联合多糖铁复合物对血液透析后贫血患者血液生化指标及铁代谢指标的影响

摘要 目的探究左卡尼汀联合多糖铁复合物对血液透析后贫血患者血液生化指标及铁代谢指标的影响.方法选取2020年2月—2022年1月本院收治的60例血液透析后贫血患者,采用随机数字表法分为两组,各30例.对照组行贫血纠正的常规治疗联合多糖铁复合物,联合组在对照组基础上联合左卡尼汀注射液.对比两组治疗效果、血清血红蛋白(Hb)、血细胞比容(Hct)、红细胞计数(RBC)、血清铁(FE)、总铁结合力(TIBC)及铁蛋白(FERR)及不良反应.结果联合组治疗总有效率高于对照组,差异有统计学意义(P<0.05).治疗前,两组血清Hb、Hct、RBC、FE、TIBC、FERR水平对比,组间差异无统计学意义(P>0.05);治疗结束时,两组血清Hb、Hct、RBC、TIBC、FERR水平均高于治疗前,FE水平均低于治疗前,且联合组血清Hb、Hct、RBC、TIBC、FERR水平均高于对照组,FE水平低于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率对比,差异无统计学意义(P>0.05).结论血液透析后贫血患者在常规治疗基础上联合左卡尼汀注射液与多糖铁复合物可显著提高疗效,改善贫血状态,纠正铁代谢异常,且安全可靠. Objective To explore effects of l-carnitine combined with polysaccharide iron complex on blood biochemical indexes and iron metabolism indexes in patients with anemia after hemodialysis.Methods 60 patients with anemia after hemodialysis admitted to the hospital from February 2020 to January 2022 were selected and divided into two groups by random number table method,with 30 cases in each group.The control group received conventional treatment for anemia correction combined with polysaccharide iron complex,and the combination group was combined with L-carnitine injection on the basis of the control group.The therapeutic effects,serum hemoglobin(Hb),hematocrit(Hct),red blood cell count(RBC),serum iron(FE),total iron binding capacity(TIBC),ferritin(FERR)and adverse reactions were compared between the two groups.Results The total effective rate of treatment in the combination group was higher than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in serum Hb,Hct,RBC,FE,TIBC and FERR levels between the two groups(P>0.05);at the end of treatment,the serum Hb,Hct,RBC,TIBC and FERR levels in the two groups were higher than those before treatment,the FE levels were lower than those before treatment,and the serum Hb,Hct,RBC,TIBC and FERR levels in the combined group were higher than those in the control group,and the FE level was lower than that in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination of L-carnitine injection and polysaccharide iron complex in patients with anemia after hemodialysis on the basis of conventional treatment can significantly improve the curative effect,improve the anemia state,correct abnormal iron metabolism,and be safe and reliable.
