摘要
目的探讨对胃癌根治术后患者采用肠内、外营养饮食干预对其胃肠功能、营养状况及免疫功能的影响.方法选择该院2020年1—12月收治的胃癌根治术后患者80例,以随机数表法分组.对照组40例予以肠外营养支持,试验组40例予以肠内、外营养饮食干预,两组均进行营养支持7 d后,采集患者空腹静脉血检测胃泌素(gastrin,GAS)、胃动素(motilin,MTL)水平评价患者的胃肠功能,检测干预前后白蛋白、前白蛋白水平以及免疫球蛋白(immunoglobulin,Ig)M、IgA、IgG水平评价患者的营养状况及免疫功能.结果干预后,试验组GAS、MTL水平分别为(167.45±40.20)pg/mL、(231.08±38.40)pg/mL,均高于对照组,白蛋白、前白蛋白水平分别为(36.50±2.25)g/L、(172.60±13.62)g/L,均高于对照组,IgM、IgA、IgG水平分别为(1.39±0.18)g/L、(1.12±0.22)g/L、(10.74±1.58)g/L,均低于对照组,组间差异有统计学意义(P<0.05).结论对胃癌根治术后患者采用肠内、外营养饮食干预的效果理想,有利于改善胃肠激素水平及机体营养状况,提升免疫功能,值得推广.
Objective To explore the effects of enteral and parenteral nutrition diet intervention on gastrointestinal function,nutritional status and immune function in patients after radical surgery for gastric cancer.Methods From January to December 2020,80 cases after radical surgery for gastric cancer were selected and grouped by random number table.40 cases in the control group received parenteral nutrition support,and 40 cases in the experimental group received enteral and parenteral nutrition diet intervention.After 7 days of nutritional support for both groups,fasting venous blood was collected to test gastrin(GAS)and motilin(MTL)levels to evaluate the gastrointestinal function of the patients,the levels of albumin,prealbumin,and immunoglobulin(Ig)M,IgA,and IgG levels before and after the intervention were tested to evaluate the nutritional status and immune function of the patients.Results After the intervention,the levels of GAS and MTL in the experimental group were(167.45±40.20)pg/mL and(231.08±38.40)pg/mL,respectively,which were higher than those of the control group,the albumin and prealbumin levels were(36.50±2.25)g/L and(172.60±13.62)g/L,respectively,which were higher than those of the control group,the levels of IgM,IgA,and IgG were(1.39±0.18)g/L,(1.12±0.22)g/L,and(10.74±1.58)g/L,respectively,which were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The effects of enteral and parenteral nutrition diet intervention for patients after radical gastric cancer surgery are ideal,which is beneficial to improve the level of gastrointestinal hormones and the nutritional status of the body,and enhance the immune function.It is worthy of promotion.
