逆行交锁髓内钉治疗股骨远端骨折临床分析

The study of treatment of fractures in the distal of femurs with retrograde interlocking intramedullary nail

目的探讨逆行交锁髓内钉内固定治疗股骨远端髁上、髁间骨折的临床疗效。方法对18例股骨远端骨折患者行切开复位,逆行置入交锁髓内钉内固定,术后配合CPM行患肢关节功能锻炼。结果18例患者术后X光片显示骨折复位达到解剖复位或接近解剖复位,随访平均15个月,18例患者全部骨性愈合,无内固定物松脱或断裂,膝关节功能优良率达89%。结论逆行交锁髓内钉内固定治疗股骨远端骨折,在保证骨折解剖复位的前提下,达到坚强内固定,能早期行关节功能锻炼,是一种非常有效的治疗方法。

Objective To investigate the clinical effect of retrograde interlocking intramedullary nail in the treatment of supracondylar or intercondylar fractures of femurs.Methods 18 cases of fractures in the distal part of femurs were applied with open reduction and retro-interlocking nail fixation .All patients were encouraged to perform functional excises of knee joint under CPM assistance after operation.Results In the follow up of 15 months,X-ray manifested that all fractures were bony healing and reduced to anatomical or nearly anatomical position.No loosing or break occurred,the excellent rate of knee function was 89%.Conclusion Retro-interlocking intramedullary nail fixation is an alternative method in the treatment of distal femoral fracture,it can offer significant advantages including anatomical position,stable fixation,and early functional recovery.
