摘要
目的调查临床主要病原菌的分布及两种主要非发酵菌耐药性的变化。方法回顾分析2007~2010年临床分离的病原菌和两种主要非发酵菌的耐药性。结果医院常见病原菌主要为铜绿假单胞菌(16.0%~20.7%)、大肠埃希杆菌(11.7%~18.4%)、鲍曼不动杆菌(10.4%~14.2%)、肺炎克雷伯杆菌(12.2%~16.7%)、白假丝酵母菌(6.0%~9.6%)和葡萄球菌(3.7%~11.7%);铜绿假单胞菌和鲍曼不动杆菌对碳青霉烯类抗生素亚胺培南耐药性呈现增强趋势,分别为47.3%~70.9%,12.6%~45.0%。耐头孢哌酮/舒巴坦的鲍曼不动杆菌年年上升,由2007年的11.9%上升到2010年的25.0%。结论铜绿假单胞菌和鲍曼不动杆菌是医院感染的常见菌,对碳青霉烯类抗生素及头孢哌酮/舒巴坦耐药较严重,医师应依据检验结果个性化用药,医院需采取有效措施,降低碳青霉烯类抗生素及头孢哌酮/舒巴坦耐药性。
Objective To investigate the distribution of main microbial pathogens and the drug resistance of two main kinds of non-fermenting bacteria in hospital.Methods To retrospectively analysis the kinds of main microbial pathogens and the drug resistance of two main kinds of non-fermenting bacteria in Taian Hospital from 2007 to 2010.Results The main microbial pathogens in hospital were Pseudomonas aeruginosa(16.0%-20.7%),Escherichia coli(11.7%-18.4%),Acinetobacter baumannii(10.4%-14.2%),Klebsiella pneumoniae(12.2%-16.7%),Candida albicans(6.0%-9.6%) and Staphylococcus(3.7%-11.7%).The resistance of carbapenem antibiotics on Pseudomonas aeruginosa and Acinetobacter baumannii were growing up,the rate of Imipenem resistance on Pseudomonas aeruginosa and Acinetobacter baumannii were from 47.3% to 70.9% and from 12.6% to 45.0% respectively.The resistance of cefoperazone/sulbactam on Acinetobacter baumannii increased yearly,it grew up from 11.9% in 2007 to 25.0% in 2010.Conclusions Pseudomonas aeruginosa and Acinetobacter baumannii are the common bacteria in hospital.The resistance rate of carbapenem antibiotic and cefoperazone/sulbactam is high and increase yearly.Doctors should administrate patients individualy according to the results of laboratory and the effective treatments should be used in hospital.Need to take to control,thus,it can decrease the resistance rate of carbapenem antibiotic and cefoperazone/sulbactam in clincal.
