摘要
目的探讨荧光原位杂交法(FISH)及全自动图像细胞法(ICM)对膀胱肿瘤的诊断价值。方法对20例非尿路上皮癌和40例膀胱肿瘤患者的尿液采用全自动图像细胞法及FISH、进行尿细胞学检查。另对确诊的45例膀胱肿瘤患者肿瘤组织及10例排除膀胱肿瘤患者的正常膀胱黏膜,行流式细胞VEGF及VEGFR检测。结果 FISH的敏感性显著高于ICM的敏感性(P<0.05)和常规尿细胞学的敏感性(P<0.05),同时ICM敏感性也显著高于常规尿细胞学的敏感性(P<0.05);ICM、FISH和常规尿脱落细胞学检测的敏感性与患者病理分期无相关性(P>0.05),但与肿瘤病理分级显著相关(P<0.05)。结论 FISH对膀胱尿路上皮癌诊断的敏感性高于ICM和尿细胞学检查。VEGF、VEGFR定量分析可作为膀胱肿瘤的生物学行为评估方法之一。
Objective To study the application of fluorescence in situ hybridization and Automated Image Cytometry in the diagnosis of urothelial carcinoma of bladder and the expression of VEGF and VEGFR .Methods 60 patients were divided in-to two groups ,40 patients were diagnosed as urothelial carcinoma of bladder by pathology and 20 healthy individuals as control group.2Urine was collected and tested by ICM ,2FISH,cytology.The tissue of bladder cancer from 45 patients who have been as diagnosed bladder cancer and the mucosa of normal bladder was tested by FCM .Results Overall sensitivity of FISH was signifi-cantly higher than that of ICM (P<0.05) and urine cytology (P<0.05),and ICM was more sensitive than urine cytology (P<0.05).Sensitivities of ICM,FISH and urine cytology had significant correlation with grade (P<0.05).Conclusion FISH have significantly higher sensitivity in diagnosis of urothelial carcinoma of bladder than ICM and urine cytology .VEGF and VEGFR may be a new method in evaluation of bladder cancer .
