急诊胃镜注射组织胶治疗胃底静脉曲张破裂出血疗效观察

Endoscopic Injection of Tissue Glue(GLUBRAN2) in The Treatment of Gastric Fundus Varices

目的本文重点评价GLUBRAN-2在治疗胃底静脉曲张出血中的疗效。方法 12例确诊为胃底静脉曲张出血的患者在出血发生24 h内接受胃镜下注射GLUBRAN-2,观察包括即时止血率、早期再出血率、治疗相关并发症,与再出血和死亡相关的肝功能Child分级、曲张静脉形态、预后、肝硬化病因等指标。结果 12例活动性出血的患者均达到即时止血,无再出血,未发生栓塞并发症,3例发生低热,1例死于出血后肝功能衰竭。结论急诊胃镜下注射GLUBRAN-2治疗胃底静脉曲张破裂出血疗效显著,并发症发生率低。

Objective GLUBRAN2 is a synthetic cyanoacrylic surgical glue modified by addition of a monomer synthesized by the manufacturer.It has outstanding haemostatic and adhesive properties,and once solidified.It was recommended use in treating gastric fundus bleeding in Europe.To evaluate the efficacy of tissue glue(GLUBRAN2) in the treatment of gastric fundus varices.Methods:12 patients with gastric fundus varices were received endoscopic injection of tissue glue.All had active bleeding.Endoscopy were done in 24 hours after bleeding.Among 12 patients were follow -up and the median duration was 4 months.Results All 12 emergency injection stopped bleeding successfully.No one had rebleeding,no embolism happened.3 patients had low grade fever.1 patient died due to liver failure.Conclusion Endoscopic injection of GLUBRAN2 is a effective method for gastric varices with high initial haemostasis rate and low major complications.
