协调牙弓宽度角度早期阻断性治疗偏颌畸形伴下颌后缩

摘要 目的:评价通过早期协调上下颌牙弓宽度,阻断性治疗青少年偏颌畸形的临床疗效。方法:选择8例青少年偏颌畸形伴下颌后缩患者,采用活动式上颌牙弓缩弓器竖直上后牙,同时配合下颌固定矫治器排齐下后牙,应用适当的颌间牵引调整上下颌咬合关系,矫治前后拍摄正位片和头颅侧位片,并进行X线头影测量分析。结果:①矢状方向上,SNA增加(0.7±0.9)°,SNB增大(2.4±1.7)°,ANB减少(1.7±0.9)°,颌骨前后向不协调减轻;前牙覆盖减少(3.8±1.0)mm。②垂直方向上,MP-FH增加(1.1±2.6)°,矫治前后差异无统计学意义(P>0.05)。③术后上下颌中线均与面中线对齐。结论:早期协调上下颌牙弓宽度能有效矫治青少年偏颌畸形,有助于青少年患者的下颌骨生长发育。 Objective To evaluate the effect of the interceptive treatment of the asymmetric malocclusion by coordinating the transverse relationship.Method 8 adolescents were selected(3 males,5 females,aged from 11.4-13.2 years).They were all treated with removable upper arch contractor and straight wire technique.Cephalometric radiographs were taken and analyzed at the pre-treatment(T1)and post-treatment(T2).Students′-test was used to determine if there were significant differences between the 2 periods.Results SNA increased by(0.7±0.9)°,SNB increased by(2.4±1.7)°,and ANB decreased by(1.7±0.9)°,anterior overjet decreased by(3.8±1.0)mm.Mp-FH increased by(1.1+2.6)°,which had no significant difference.Conclusion To coordinate transverse relationship in the early stage is a clinically effective method for correcting the asymmetric malocclusion.
