Psychological Feelings for Patients with Shrinking Man Syndrome Complicated with Hemodialysis: A Qualitative Research

目的探讨和了解血液透析合并退缩人综合征(shrinking man syndrome,SMS)患者的真实心理感受,以引导护士为患者提供更为有效的护理干预措施。方法采用现象学研究方法探讨5名血液透析合并退缩人综合征患者的心理感受,以半结构式的深入访谈法取得访谈资料,对访谈资料进行缩减、分类、理解、诠释和分析,提炼主题,同时采用治疗性持续沟通技巧,加强与患者及家属的交流。结果血液透析合并退缩人综合征患者的心理感受主要有:患者对家属的愧疚心理,出现社交退缩现象,形象改变导致自卑易怒心理,渴望家人陪伴及精神支持,对治疗效果信心不足。结论护士应了解血液透析合并退缩人综合征患者的心理状态,加强有针对性的心理支持和心理护理,以提高患者的生活质量。

Objective To explore and understand the psychological feelings for patients with shrinking man syndrome complicated with hemodialysis and to guide nurses to provide them wholehearted psychological nursing interventions.Methods Phenomenological research methods were used to investigate the psychological feelings of the five patients,and then shorten,classify,understand,refine the interview data which were obtained in semi-structured in-depth interview.The therapeutic continuous communication skills were adopted to enhance the communication with patients and their families.Results The psychological feelings of patients with shrinking man syndrome complicated with hemodialysis were as follows: patients' guilty for the families,social withdrawal,inferiority and irritability following image changing,desire for the accompany by family members and spiritual support and lack of confidence in the treatment effect.Conclusion Nurses should know the psychological feelings for patients with shrinking man syndrome complicated with hemodialysis,strengthen the psychological support and life care,so as to improve the quality of life.
