老年人巨大十二指肠憩室——附3例报告及文献复习

Giant duodenal diverticulum in elder: report of 3 cases and review of literature

目的 探讨老年人巨大十二指肠憩室的临床表现、诊断和治疗手段。 方法 回顾性分析3例老年巨大十二指肠憩室患者的临床资料,并复习有关文献。 结果 3例中男1例,女2例,年龄分别为75、82和68岁,临床症状有腹痛、恶心、呕吐、黑便和波动性黄疸等。胃肠钡餐和CT提示十二指肠巨大憩室(直径大于10 cm)。3例均行十二指肠空肠Roux-en-Y吻合术,手术效果良好,无并发症。 结论 老年人巨大十二指肠憩室症状无特异性,当有并发症时应该采取手术治疗。手术方式以十二指肠空肠Roux-en-Y吻合术为宜。

Objective To discuss the clinical manifestations, diagnosis and treatment of giant duodenal diverticulum in elder. Methods The clinical data of 3 elderly patients with duodenal diverticulum were analyzed retrospectively, and the literature were reviewed. Results Of 3 cases, 1 was male and 2 were female. The patients age were 75, 82 and 68 years , respectively. The clinical symptoms included abdom and inal pain, nausea, vomiting, melaena and jaundice. Barium meal and CT examination showed that there was a giant diverticulum (>10 cm in diameter) in the duodenum of 3 patients. The surgical procedure consisted of duonenojejunostomy utilizing a Roux-en-Y loop were performed in 3 cases, and the outcomes were satisfied. Conclusion There are no specific symptoms of giant duodenal diverticulum in elder. If the patients have the complications related to the disease, surgical treatment should be chosen.
