多层螺旋CT仿真内镜技术在胃癌诊断中的应用价值探讨 被引量:16

摘要 目的 探讨多层螺旋CT仿真内镜 (CTVE)检查的临床应用价值及其在胃癌诊断中的作用。资料与方法 以手术病理或电子内镜检查结果为最终判断依据 ,对 4 0例患者进行CTVE检查 ,探讨该检查手段的检查前准备 ,扫描参数 ,三维后处理方式及其与原始图像相结合对诊断结果的影响 ,比较CTVE的肿瘤显示与最终诊断的符合程度及各种重建方式对胃癌显示的有效性。结果 CTVE检查对胃癌诊断的敏感性为 97% ,特异性为 75 % ,准确性为 95 % ,漏诊率为 3% ,误诊率为 2 5 % ,阳性和阴性预测值分别为 97%和 72 %。 Objective To evaluate multi-slice CT virtual endoscopy (CTVE) in the clinical diagnosis of gastric carcinoma.Materials and Methods CTVE was performed in 40 patients with suspected gastric carcinoma. CTVE findings were compared with pathologic and electron endoscopic results. The preparation, scanning parameters and 3D post-processing technique were discussed. The diagnostic consistency between CTVE and endoscopy or pathology was assessed. The effect of different image reconstruction on the visualization of the lesion was evaluated. Results The sensitivity, specificity, correctness, missed diagnosis and misdiagnosis of CTVE for gastric tumors were 97%, 75%, 95%, 3% and 25%, respectively. The positive predictive value and negative predictive value of CTVE were 97% and 72%, respectively.Conclusion Multi-slice CTVE with multiple reconstruction imaging and combination with original image is of great value in clinical diagnosis of gastric carcinoma.
