摘要
目的 :探讨白及丹参溶胶局部涂抹联合大承气汤灌胃对大鼠损伤性腹腔粘连的预防作用机理。 方法 :采用钳夹肠管的方法制作大鼠腹腔粘连模型 ,观察肠损伤表面直接涂抹白及丹参溶胶 ,术后灌胃大承气汤对模型动物腹腔粘连程度、粘连率、血清内毒素、SOD、损伤肠段羟脯氨酸含量、肠组织NF -κB表达以及损伤肠段病理改变的影响。 结果 :与其他处理组相比 ,直接涂抹 +大承气汤组粘连程度及粘连率显著降低 (P值均小于 0 0 5 ) ;血清内毒素、SOD显著降低 (P值均小于0 0 1) ;损伤肠段NF -κB表达明显降低 ,肠组织病理损伤明显减轻。 结论 :白及丹参溶胶局部涂抹结合大承气汤灌胃 ,可在局部形成物理屏障、减轻损伤肠段局部的炎症反应 ;促进术后肠管运动 ,降低血清内毒素水平 ,减少腹腔粘连的形成。
Objective To investigate the therapeutic mechanism of Baiji Danshen solution used locally combined with Dachengqi Decoction given orally on intraperitoneal adhesion in rat. Methods Thirty Wistar rats were randomly allocated to five groups:A.sham operation,B.adhesion model,C.topical application of Baiji Danshen solution.Intraperitoneal adhesions were induced by clamping the duodenum in B,C.D and E groups followed by painting dirsctly on the damaged serosal surface of the duodenum with Baiji Danshen solu'in C and D groups and with saline in B and E groups.After operation,Dachengqi decoction was administered orally in D and E groups(2ml/100g body weight,bid).Five days later,adhesion acores and adhesion incidence were evaluated.The levels of endotoxin,superoxide dismutase(SOD) of serum and the levels of oxoproline in damaged intestinal tissue were detected.Western blot method was employed to examine NF-k B activation of damaged intestinal tissue.The histopathological changes of the damaged intestinal tissue were observed under light microscopy.Results In group D,the severity of adhesion and the adhesion incidence were all reduced significantly than those in groups B.,C and (P<0 05).The levels of endotoxin,SOD and oxoroline were all reduxed in group D than thise in groups B and C.The NF-k B actication of the damaged intestinal tissue was also attenuated significantly in group D compared with group B (P<0 001).The pathological injury of the damaged intestinal tissue was ameliorated obviously in group D compared with that in group B. Conclusion Painting Baiji Danshen solu' directly on the damaged surface of the duodenum combined with application of Danshen secoction orally could effectively reduce the severity and incidence of intraperiponeal adhesion,possibly by suppressing the early stage of inflammation,protecting the damaged serosal surface of intestine,promoting intestinal movements and reducing the serum entotoxin level.
