摘要
目的:观察轻度认知障碍(mildcognitiveimpairment,MCI)患者电刺激小脑顶核后脑血流的变化情况,探讨慢性脑缺血与认知功能损害的相关性。方法:武汉大学中南医院神经科门诊与病房的MCI患者30例,符合美国Mayo神经病学研究中心提出的MCI诊断标准及国内张明圆等根据文化程度对痴呆的评分标准,无心脑血管疾病史,无神经功能缺损。30例MCI患者接受脑循环功能治疗仪治疗10d,治疗前后行经颅多普勒(TCD)检查并对结果进行相应的统计学分析。结果:MCI患者治疗前TCD检查异常率为90%(27/30),治疗后大脑中动脉和基底动脉脑血流速度为(58.7±9.9),(38.7±8.1)cm/s,比治疗前犤(43.6±8.9),(28.6±5.1)cm/s〗有明显改善,差异有显著性意义(t=7.369,5.376,P<0.05)。结论:电刺激小脑顶核能增加MCI患者脑血流量,可能对MCI患者的认知有所改善。
AIM:To observe the changes of cerebral blood flow in patients with mild cognitive impairment(MCI) after electrostimulation to the cerebellar fastigial nucleus,so as to study the correlation between chronic cerebral ischemica and cognitive impairment. METHODS:Thirty MCI patients in the outpatient and sickrooms of Department of Neurology,Central South Hospital of Wuhan University,who were in accordance with the diagnostic criteria of MCI raised by American Mayo Research Center of Neurology and the scoring standards of dementia according to the educational levels by Ming-Yuan Zhang,et al.in China,were involved in the study,and they had no the history of cerebrovascular disease and neurological impairment.All the patients were treated with the cerebrovascular function machine for 10 days.They were examined with transcranium Doppler(TCD)before and after treatment,and then the results were analyzed statistically. RESULTS:The examination of TCD showed that the abnormal rate of the MCI patients was 90%(27/30).Cerebral blood flow of middle cerebral artery and basilar artery in MCI patients after treatment were(58.7±9.9),(38.7±8.1) cm/s,significantly ameliorated as compared with those before treatment [(43.6±8.9),(28.6±5.1) cm/s](t= 7.369, 5.376,P< 0.05). CONCLUSION:Cerebral blood flow of MCI patients can be increased by electrostimulation to cerebellar fastigil nucleus, and perhaps ameliorate the cognition of MCI patients.
